About the Role:

This is a rare opportunity to become part of our team. We have not recruited case workers within the Elderly Care Unit for several years, which we see as a testament to a stable workplace with good morale. The position is now open as a valued colleague moves on to a new assignment within the same administration. The Elderly Care Unit belongs to the Authority Division of the Social Services Administration. Currently, ten case workers work within the Elderly Care Unit, along with a fee case worker, a case worker for transport services and national transport services, and a housing coordinator. The unit is led by a group manager and a unit manager who together are responsible for developing and supporting the operations.

Here, you will become part of a dedicated and experienced team working to provide elderly residents with professional, legally secure, and individualized support. At the same time, we face an exciting future where the new Social Services Act and new working methods create opportunities to further develop the operations.

Responsibilities:

As a case worker in elderly care, you are responsible for case processing, investigation, decisions, and follow-up according to the Social Services Act (SoL) for persons aged 65 and older, as well as for persons under 65 with dementia. Decisions are made according to the current delegation order, where case workers have a comprehensive decision-making mandate within the unit's area. You meet individuals with different needs and life situations and conduct investigation interviews in the individual's home, at the hospital, or in our facilities. To create more time for professional meetings and reduce administration, we explore and develop new digital working methods, including AI-based solutions, such as support in documentation work.

The work also involves close collaboration with relatives, providers, caregivers, and other partners. You are independently responsible for your cases from application to decision and follow-up, and you actively contribute to the unit's development and improvement work. At the same time, collaboration and knowledge sharing are important parts of our working methods. We have regular case reviews and case guidance where we together discuss complex issues, create consensus, and contribute to a learning organization.

Qualifications:

We are looking for you who have:

• A degree in Social Work or another higher education qualification that the employer considers equivalent.
• At least two years of continuous experience in authority practice with independent case processing, investigation, decisions, and follow-up according to the Social Services Act (SoL) and/or the Act on Support and Service for Certain Disabled Persons (LSS).
• Experience in working based on evidence-based investigation methodology, such as IBIC (Individual's Needs in Focus) or corresponding method support.
• Very good ability to express yourself in Swedish, both orally and in writing.
• Valid B driver's license.

Merits:

• Experience in case processing and investigation according to the Act on Transport Services and National Transport Services.
• Experience in municipal operations and politically directed organization.
• Experience with the Treserva system for case processing and documentation.
