Do you want to make a difference for patients with cardiovascular disease and contribute to good and preventive care? We are now looking for an engaged and independent nurse for our blood pressure clinic at our health center.

Health Center Centrum

Health Center Centrum is an established operation with approximately 13,000 registered patients, making us one of the larger health centers in Skåne. Here, we have a combined and versatile competence including nurses, doctors, psychosocial competence, and rehabilitation staff. Together with nursing assistants and medical secretaries, they form the foundation of our operations. The operation also includes a child health center as part of a family center.

Job Responsibilities

As a nurse at the blood pressure clinic, you are responsible for follow-up, counseling, and treatment of patients with hypertension and other cardiovascular risk factors. The work involves both independent patient visits and close collaboration with doctors and other professions within primary care.

Your duties include, among other things, clinic visits, follow-up of patients with hypertension, blood pressure checks, and risk assessments. You also work with patient counseling regarding lifestyle habits, self-care, and medication treatment, as well as telephone counseling and digital patient contacts. Duties also include documentation and quality follow-up according to current guidelines, as well as collaboration with other professional categories within the health center. At our workplace, there is an opportunity for further education and competence development. As a new employee, you will receive an individual introduction adapted to your experience.

Qualifications

We are looking for you who are a licensed nurse with the right to practice in Swedish healthcare. Language skills in Swedish equivalent to at least level C1 according to the Council of Europe's scale are required. Experience working in primary care, internal medicine, and/or cardiovascular care is meritorious.
