We are looking for a temporary assistance case manager with a focus on SoL - Elderly Care.

About the workplace

The Assistance Unit consists of 7 assistance case managers who investigate according to the Social Services Act (SoL) and 3 assistance case managers who investigate according to the Act on Support and Service for Certain Functional Impairments (LSS), a team leader and a unit manager. At the Assistance Unit, we strive for equal assessments and to maintain high legal certainty. We work in teams and are unpretentious. As an assistance case manager with us, you have access to support in the case processing process from colleagues, team leader and unit manager. We collaborate, among other things, with other units within individual and family care as well as providers.

Job responsibilities

Your work is to enable a legally secure case processing through qualified investigations and decisions made according to the Social Services Act (SoL) as well as to be responsible for follow-ups and reviews of decisions. An important part of your work is a good citizen focus. This means that you collaborate with both external partners within health care, relatives and close ones as well as internal partners and other social services. This requires good communication skills and a professional working method. The work as an assistance case manager also includes informing and guiding citizens to the right support and measures.

In addition to the case processing with decision-making, job responsibilities include documenting, collaborating both externally and internally and participating in the unit's scheduled meetings and training. We work in the business systems Pulsen Combine and Cosmic Link, which we prefer to see that you have previous experience with. We work based on the working method Individual Needs in Focus (IBIC).

Qualifications

We are looking for you who are a social worker, have a social care exam, a bachelor's degree in social care or another adequate education that the employer considers suitable. You should have previous experience of authority exercises within SoL and experience of the IBIC working method.

As a person, you are unpretentious, quick to learn and confident in yourself and have a good ability to prioritize. You are flexible and ensure that what needs to be done gets done. You see more opportunities than difficulties and enjoy challenges in finding constructive solutions to complex problems. You are humble and receptive to others' opinions. You have an easy time collaborating both internally and externally. Good treatment is a matter of course for you.

You need to be well versed in current legislation and have the ability to work with complex issues. For the position, it is required that you have a good command of the Swedish language and the ability to express yourself well both orally and in writing.

Great importance is attached to personal suitability and a professional attitude.
