Activity and Collaboration Coordinator in Habo Municipality

We are looking for an engaged and relationship-building Activity and Collaboration Coordinator for Habo Municipality.

You will play a vital role in strengthening social cohesion, preventing ill health, and reducing involuntary loneliness among the elderly and individuals within the disability sector.

The work primarily takes place at Träffpunkt Habo, but also in other environments within the municipality, in collaboration with the voluntary sector and civil society, as well as through outreach work which may include home visits. Evening work may occur in connection with activities and collaboration.

Your closest colleague will be a next-of-kin coordinator, and you will also belong to a unit consisting of 8 social secretaries and a unit manager.

Job Responsibilities

The position involves significant personal responsibility and independent work. As an Activity and Collaboration Coordinator, you will have a varied role where tasks may change over time based on the needs of the operations and individuals. You are expected to independently plan, structure, and drive the work forward, as well as contribute to developing new working methods and models within the operation. The position includes, among other things:

• Preventive social work within the elderly and disability sectors
• Creating and implementing activities at Träffpunkt Habo and other venues
• Outreach work and supportive measures, including home visits
• Coordinating volunteers and developing waiting services
• Building and strengthening collaboration with associations, civil society, and municipal operations
• Conducting health conversations and referring patients when necessary
• Working with information and communication regarding the operation

Qualifications

Requirements

• Social work degree or other education assessed by the employer as equivalent
• Good ability to express yourself in Swedish, both orally and in writing
• Good ability to create and develop relationships and collaborate with various actors

Employment type: Permanent position.

The position is during daytime hours, but evening work may occur.
