The home care service is looking for someone who wants to work with planning within home care.
Job Responsibilities
You will work with a computerized planning system, Life Care, where you plan the approved services that home care staff perform for clients. You will be responsible for the client-facing and daily work through daily planning in the Life Care system. The job involves close collaboration with the unit manager, home care staff, as well as contact with clients and relatives.
You are responsible for planning based on set goals, such as continuity, contact management, economy, and work environment, etc. You will also receive and schedule new cases and be responsible for following up on client cases and updating changes in needs and services.
The job also involves placing substitute requests to the staffing unit and, in consultation with the unit manager, planning for absences such as vacations. Your tasks also include some documentation within social documentation.
The position is a summer temporary position.
Qualifications
You are accustomed to using a computer as it will be your main working tool. It is advantageous if you have experience and knowledge from operational support systems such as Life Care, BeSched, and Phoniro.
As a person, you are meticulous, proactive, and responsible. You also have good collaboration skills and can structure your work approach. You are flexible and can build trusting relationships and want to contribute with your positive spirit and enthusiasm in the work. You have an economic awareness.
We are looking for someone who has:
- At least a high school education.
- Good computer skills.
- A solution-focused approach to work as no day is the same.
- A driver's license is desirable as transfers between units may be required.
- We prefer that you have work experience in elderly care, home care, support and care, or personal assistance.
- Experience from similar assignments is an advantage.
- Good knowledge of the Swedish language, both spoken and written.
We offer scheduled working hours; day and weekend work every fourth week. The municipality applies a smoke-free working time.
Conditions
To be considered for employment, you may be required to, in connection with the interview or before employment:
- present valid ID
- if necessary, provide proof of education with grades or diploma
- have the right to work in Sweden and be able to present a valid work permit
- if necessary, present a valid driver's license
- if necessary, present a valid professional license
For certain positions, a security check may be conducted in accordance with applicable regulations. If the position is subject to a security check, you must undergo and be approved in this. For a few security-checked positions, in addition to the requirement for a passed security check, Swedish citizenship may also be required.
