To meet the residents' needs for more accessible and close healthcare, Region Skåne has initiated a new form of care in collaboration with existing healthcare providers. We have established a mobile team that strengthens care and offers safe and qualified care at home for patients who prefer this over hospital care. The team acts as a coordinating hub and is responsible for assessing, planning, prescribing, and performing interventions directly in the patient's home. Care is tailored to each patient's individual needs, aiming to provide both medical and psychosocial qualified care.

The operation started in January 2023, and the team consists of doctors, nurses, social workers, and medical secretaries. We operate from our premises at the Hospital in Landskrona and are staffed every day between 8 AM and 5 PM. On weekdays, we work from Landskrona, while weekend work is done in collaboration with colleagues in Helsingborg and Ängelholm.

The positions are full-time but with the possibility of part-time.

JOB RESPONSIBILITIES

As a specialist doctor with us, you have a central role in performing medical assessments and interventions directly in the patient's home or in special accommodations, together with a nurse. You actively participate in planning and following up on care interventions in close collaboration with the team and other healthcare providers. Together, we work to create safe and cohesive care for our patients.

Your main tasks include making medical assessments, providing prescriptions, and following up on patients with complex care needs. In addition, you contribute to the development of the operation by participating in improvement and development work.

QUALIFICATIONS

We are looking for you who are a licensed physician with the authority to practice within Swedish healthcare and who has specialist competence in general medicine, geriatrics, internal medicine, or another equivalent specialty. You have good knowledge of the Swedish language, corresponding to at least level C1 according to the Council of Europe's level scale. A valid B driver's license is a requirement as care is mainly provided in patients' homes.

As a person, you are confident in your professional role, have a good ability to assess care levels in sometimes seriously ill patients, and always put the patient first. You enjoy working in a team, are communicative, and have the ability to meet patients and relatives in various life situations. You have a flexible approach and can adapt to the needs of the operation. Great emphasis is placed on personal suitability.

This includes, among other things, mobile teams and specialized palliative care, outpatient care within close care, emergency care within close care, 1177 by phone, and some region-wide functions. In addition, the management is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
