In Trelleborg, we work together for a growing successful community where it is good to live. Our 61 villages and 21 districts are creative meeting places that bubble with life. Here we want things, tell about them, and come just as we are. This makes us welcome others in the same generous and curious way.
Welcome to take your place on the journey!
The Social Administration's mission is to work with social services and provide support so that everyone in the municipality feels safe and has the same opportunities in life. We want everyone to be able to be active and involved in society. With a focus on respect for the individual's right to decide over their own life, we are responsible for three important areas: care and support, assistance for people with disabilities, and help for individuals and families. Our goal is for everyone to receive the support and care they need.
Since everything Trelleborg Municipality does is based on our values of openness, respect, and responsibility, it is important and crucial that these values feel as self-evident to you as they are to us in our work.
The Workplace
As an employee with us, you will be part of the Nursing Unit in Trelleborg Municipality, a competent and supportive operation where nurses, specialists, and other professionals work together to create safe and person-centered care.
Here we work closely together and have a climate characterized by collaboration, collegial support, and a shared commitment to developing the municipality's care and support.
Your physical base will be at the Nursing Unit in Trelleborg. Here you will closely collaborate with, for example, MAS/MAR, family support, and case managers — important partners in daily work. The role involves both on-site work, home visits within the municipality, as well as contact via phone and digital tools, providing good variation and flexibility in your working methods.
We are in a development phase, where both the national dementia strategy, new social services legislation, and the transition to good and close care create opportunities to shape the future of dementia work. With us, you will make a concrete difference for people with cognitive diseases and their relatives.
We Offer You
- A varied and independent job where you can use your specialist competence
- Individually tailored introduction, providing a secure start in the assignment
- Opportunity for vacation exchange with favorable conditions
Job Responsibilities
In this role, you will serve as a central knowledge resource in the municipality's dementia work. You will provide support and guidance to individuals with cognitive diseases, their relatives, and staff. The work is characterized by a person-centered approach and is based on creating good cooperation among all involved parties.
You will work both independently and together with other professionals. The assignment includes mentoring and training staff and, if necessary, participating in patient-related meetings. You will work from a holistic perspective where coordination in the care chain is of great importance.
Collaboration occurs regularly with primary care, the region, case managers, family support, and other relevant actors. You will contribute specialist competence in team meetings and participate in the establishment of a Coordinated Individual Plan (SIP).
An important part of the role is also to actively participate in development work through environmental monitoring, identifying areas for improvement, and proposing solutions. You will also participate in professional networks to ensure that the operation maintains a high and current level of knowledge.
Qualifications
We are looking for a licensed nurse with specialist training in dementia, elderly care, or other equivalent education. You have several years of experience working in the dementia field and a clear, documented commitment to the target group. You possess good clinical experience and are familiar with working with the BPSD registry.
You have experience in mentoring and training staff and feel confident in an advisory and supportive role towards both staff and partners.
A B driver's license is required for the position.
Desirable qualifications include: Certified BPSD trainer and knowledge and experience with the operational system Life Care.
We place great emphasis on your personal qualities. To succeed in the role, we see that you are:
- Confident and pedagogical, with the ability to guide others.
- Relationship-building and skilled at creating trusting contacts.
- Collaborative, with a good ability to work in teams and contribute specialist knowledge.
- Structured and independent, with a professional and person-centered approach.
- Able to see the whole picture regarding the person with cognitive disease and coordinate efforts accordingly.
- Development-oriented, engaged, and active in improvement and quality work.
