Your mission

We are looking for an engaged and responsible case officer who wants to make a real difference for the elderly and people with disabilities in our municipality.

With us, you will play an important role in the work of investigating, deciding, and following up on assistance and interventions according to the Social Services Act (SoL) and the Act on Support and Service for Certain Disabled Persons (LSS). Together, we develop our operations and actively contribute to the vision and community development of Söderhamn.

In our work, we use IBIC (Individual Needs in Center), a structured and needs-oriented approach that ensures legally secure processing and clear documentation.

As a case officer with us, you will have a central and significant role where you:

  • Meet people in different life situations and build trusting relationships.
  • Investigate and assess the need for various forms of home care services, care and nursing homes, day activities, and other individually tailored support measures.
  • Make independent and legally secure decisions according to applicable legislation.
  • Follow up and evaluate granted interventions to ensure that the right support is provided at the right time.

Description of the workplace

The Care Unit is part of the Authority Department within the Welfare sector, where we collaborate with the Supply Unit and the Adult Unit. Our unit consists of an engaged team of case officers, LSS and case officers, housing coordinators, specialists, and a unit manager.

Within the Welfare sector, about 1100 employees meet people in different life situations through their professional roles in, for example, care and nursing homes, home care, home health care, LSS housing, social psychiatry, addiction centers, and supply units.

Your competence

We are looking for someone with a university degree in social work or another relevant education that the employer assesses as equivalent. You have good knowledge of authority operations and a solid understanding of applicable legislation.

Experience as a case officer or LSS officer is an advantage. It is also beneficial if you have experience with the Lifecare operational system and knowledge of working according to IBIC (Individual Needs in Center).

You have a good ability to express yourself in Swedish, both orally and in writing. Since travel is included in the position, a B driver's license is required.

You are happy to share your knowledge and experiences with colleagues and actively contribute to a positive work environment. Furthermore, you are a good analyst who is skilled at seeing connections and solving complex questions and problems. It is also important that you can plan, organize, and prioritize your work effectively, as well as set and adhere to deadlines.
