Care Coordinator for the Medical Area in Bollnäs

The Medical Area in Bollnäs is responsible for both acute and planned internal medicine specialist care for the residents of Southern Hälsingland. The hospital has three wards, a cardiology ward, nephrology/internal medicine, and stroke/internal medicine, as well as an intensive care unit. We operate based on a person-centered approach and a process-oriented working method. Patient safety, collaboration, and a good working environment are fundamental pillars in everything we do.

Our care coordinator is retiring, and we are looking for someone who wants to continue the work of ensuring a safe and secure discharge process for our residents!

JOB RESPONSIBILITIES

As a care coordinator, you are a central function in the care chain and work across all inpatient care units and, if necessary, also in the emergency department at Bollnäs Hospital. The care coordinator's tasks are to promote and facilitate contact between providers within inpatient care, primary care, municipalities, and patients/family members. The work must be based on the laws and regulations governing healthcare, as well as current routines and collaboration agreements within Region Gävleborg.

As a care coordinator with us, you will work with:

• ensuring that the care chain and discharge process function smoothly
• working according to applicable laws, regulations, routines, and collaboration agreements
• participating in local and regional collaboration groups
• contributing to the development and quality assurance of the care planning process
• introducing and supporting staff in the Life Care system
• mentoring new staff in the care process
• if your basic profession allows, we welcome active care work at a certain percentage.

QUALIFICATIONS

You are self-driven and take responsibility for moving your work forward while being service-oriented and engaging, respectful, and attentive to those you meet. You have a solution-focused working style and a genuine interest in contributing to safe and cohesive care for patients.

In the role of care coordinator, you should have:

• a university degree in a relevant field.

It is an advantage if you have experience working as a case officer, knowledge of the Social Services Act (SoL), and/or experience in collaboration between different care providers.
