Strömsbro Din hälsocentral is looking for a Social Worker/Psychiatric Nurse

Strömsbro Din hälsocentral has about 45 employees with various professional backgrounds and currently has over 11,000 registered patients. We provide primary care with an acute team, medical consultations, district nurse consultations, home visits, child health care, psychosocial team, occupational therapy, and physiotherapy. We have specialist clinics for diabetes and asthma/COPD. We work cross-professionally within the health center and have both cross-professional and professional group meetings. We are now looking for a new team member for our psychosocial team.

JOB RESPONSIBILITIES

Often, we in primary care are the first point of contact for patients with mental health issues. This involves a lot of collaboration with other medical and rehabilitation staff at the health center. As a social worker/psychiatric nurse, you will be responsible for psychosocial assessments and short-term treatment interventions within the general medical competence area for conditions such as depression, anxiety, stress-related illnesses, crises, psychosomatic conditions, etc., for patients aged 18 and older. You will work with needs-based treatments at the individual and/or group level. Our patients also have access to internet-based cognitive behavioral therapy if deemed an appropriate treatment form. The work is independent, and competent colleagues are available for consultation as needed. We are part of the Min vård Gävleborg service, which creates opportunities for digital visits to facilitate communication between care and patients. This position complements the current care offerings and is an important piece of the puzzle to create more accessible and equitable care.

QUALIFICATIONS

You should also be accustomed to working in teams with all the health center's professions when needed around the patient.

In the role of social worker/psychiatric nurse, you should have:

• a degree in social work, a degree in psychiatric nursing, or at least KBT step 1 training
• a valid B driver's license.

It is advantageous if you have previous experience in psychosocial and psychological treatment work within primary care. It is also advantageous if you have a license as a health and medical care curator.
