Assessor for Discharge Team
The position is located within the Health and Care Administration, which is one of eight administrations in Linköping Municipality. The Health and Care Administration is responsible for the municipality's elderly care, health care, authority exercise, ordering of services, and provides care and support in its own right. Your Responsibilities

As an assessor in the Discharge Team, you will play an important role. You will manage the discharge process from inpatient care within Region Östergötland by being responsible for planning services according to the Social Services Act. The work includes close collaboration with home care, short-term accommodations, home health care, and home rehabilitation to enable a safe and secure return home.
In the role of assessor, you will inform individuals about the support available in the municipality. We work according to IBIC (Individual's Needs in Focus), a needs-oriented and systematic approach where the individual's needs and abilities are in focus.
Your Workplace
The Health and Care Administration is responsible for the municipality's elderly care, health care, authority exercise, ordering of services, and provides care and support in its own right. Each year, we provide care and support to approximately 4,100 residents of Linköping, of which about 1,350 live in special accommodations and about 2,800 receive home care. To meet the increasing number of elderly people and the rapid digital development, we are currently reviewing our organization. Our ability to change and adapt our working methods is crucial to ensure continued good and relevant care.
The administration's authority exercise for assessments is divided into two units: central unit and western and eastern unit. The central unit also includes the Discharge Team and Reception Team, and there are also resource coordinators in the unit. In total, we are about 40 employees and three unit coordinators who help prioritize and distribute the work.
The Discharge Team consists of six assessors with a strong commitment to enabling a good discharge process. You will collaborate closely with various actors in the municipality, such as home health care and external parties from the hospital. The work also includes close contact with the individual and their relatives.
Our workplace is centrally located near Linköping's travel center at Hertig Karlsgatan 4. In the autumn of 2026, we plan to move to Repslagaregatan 6. Working hours are Monday to Friday from 8 AM to 5 PM, and we apply flexible working hours.
Who We Are Looking For
We are looking for someone with a degree in social work or a degree in behavioral science, or other higher education that the employer considers equivalent. You also need to have studied social law at the university level and have experience working or interning in authority exercise.
It is an advantage if you have experience in assessments, work within elderly care, and working according to IBIC (Individual's Needs in Focus). Experience working in the journal systems Treserva and Cosmic Link is also advantageous.
You express yourself well in spoken and written Swedish, which is important as you will work a lot with documentation and user contact. You need to have a B driver's license.
As a person, you thrive in a changing environment where you are adaptable and can handle changing situations and conditions. You can also see the whole picture and take responsibility for the larger perspective in your actions. Additionally, you are self-sufficient and take great responsibility for your tasks. You are confident and can remain calm even in stressful situations. You have good collaborative skills and work well with both internal and external contacts. In your work, you will meet people of varying ages who need support in their daily lives, and it is important that you can see situations from different perspectives and distinguish between the professional and the private.
