As a Support Coordinator, you will become a key person in providing people experiencing homelessness and complex life situations with stable housing and sustainable support. You will have the opportunity to develop working methods, build long-term relationships, and see clear results of your work, both for the individual and for Piteå as a community.
Here, you will help build 'Housing First' from the ground up – a bit like jumping on a train just as it starts moving, but with more team spirit and fewer delays. You will become part of an engaged and non-hierarchical team where we help each other, share knowledge, and dare to try new ways of working. Job Responsibilities
In the role of Support Coordinator within Housing First, you support individuals who have experienced homelessness to establish themselves in their own housing and get their daily lives in order. You are the safe and attentive person who holds together the interventions around the individual, without taking over their life. The work is both practical and relationship-oriented, while maintaining a focus on legal security, quality, and structure.
You will:
- Have coordinating responsibility for the individual's support measures and serve as their fixed contact person.
- Conduct assessments and needs analyses, and develop implementation plans together with the user and their network.
- Monitor the housing situation, support contact with landlords, and other important actors in daily life.
- Collaborate closely with social services, healthcare, psychiatry, addiction care, community organizations, and other partners.
- Document systematically in our systems and contribute data to follow-up, statistics, and development work.
- Participate actively in team meetings, method development, and the launch of the Housing First operations in Piteå.
Qualifications
We are looking for you who want to combine heart, mind, and a touch of humor in everyday life. You enjoy building trust, can stand firm when things get turbulent, and thrive when working both independently and closely with others. You are curious, solution-oriented, and have no problem thinking outside the box – as long as it is legally secure and focused on the individual's best interests.
- You have relevant education with a focus on social work or psychiatry.
- You have at least 2 years of experience working with people experiencing homelessness, mental health issues, or substance abuse problems.
- It is an advantage if you have training in case management, MI (Motivational Interviewing), ESL, or similar structured working methods.
- You are confident in collaborating with public agencies and are accustomed to official contacts.
- You express yourself fluently in Swedish, both orally and in writing, and are comfortable with documentation.
- You have good digital skills and thrive in a daily life where things can change quickly and prioritization is part of the job.
