Within the Health and Care Administration, you make a difference every day by working closely with people and creating a secure and meaningful everyday life for the residents of Enköping. Together, we create safe and high-quality care and support for our citizens.

This is a fixed-term position for 1 year, and you will belong to our SoL (Social Services Act) team, which primarily meets clients in their homes or at short-stay facilities.

RESPONSIBILITIES

As a case manager, you are an important person in our operations. You work closely with people and help shape interventions that make a difference for our residents. Your work is both independent and team-based, and you will use your competence to create sustainable and legally secure solutions. Your main tasks will be to process and make decisions according to the Social Services Act.

Your responsibilities include:

• Receive applications according to the Social Services Act and investigate the individual's need for assistance
• Make decisions on interventions or rejections based on the investigation results
• Collaborate with clients, relatives, and professional actors
• Follow up and evaluate interventions to ensure quality
• Work both independently and together with colleagues in a team

QUALIFICATIONS

We are looking for you who have:

• A degree in Social Work or other education that the employer deems equivalent
• Category B driver's license
• Previous experience as a case manager is an advantage
