Social Worker with Responsibility for Reunification Work – Youth Authority

Do you want to work closely with young people and their networks, focusing on change, participation, and a safe return home? We are now seeking an engaged social worker with specific responsibility for reunification work for young people placed outside their own homes.

About the Position

You will work on cases where young people are placed outside their own homes, such as emergency foster care, residential care (HVB), and secure care (SIS). Your assignment is to, together with the responsible social worker, the young person, guardians, and partners, plan, drive, and follow up on care with a clear goal of reunification when possible and in the best interest of the child.

The work is characterized by active and structured reunification efforts where relationship, collaboration, and follow-up are central components. You will work with legal security and a strong focus on quality, participation, and long-term solutions. You will also help develop methods regarding placements and reunification.

Your Responsibilities

The role includes:

  • Following up on placements according to the Social Services Act (SoL) and the Care of Young Persons Act (LVU), ensuring care is conducted according to decisions and set goals.
  • Driving and coordinating reunification work in cases where young people are placed outside their own homes.
  • Planning and conducting follow-up and network meetings together with the young person and their network.
  • Collaborating with foster families, residential care, outpatient care, schools, healthcare, and other internal and external actors.
  • Assessing goal achievement, initiating changes, extensions, or termination of measures.
  • Documenting and making decisions in operational systems.
  • Working in accordance with applicable legislation, guidelines, and the best interests of the child.

Who Are You?

Requirements:

  • Hold a degree in Social Work (Socionom).
  • Have experience in public authority work within children and youth.
  • Have good knowledge of placements and follow-up of care outside the home.
  • Express yourself well in speech and writing in Swedish.
