The Department of Individual and Family Care in Sandviken Municipality has a functionally divided organization. Within the department's area of responsibility, there is child and youth care, addiction care, financial assistance, and family law, as well as the processing of alcohol permits and strategic work in the areas of public health and safety and ANDTS. The department has a well-developed outpatient care for children, young people, and their families, as well as for adults.

The Adult Unit is the municipality's addiction care. More than 20 people work within the unit. The core of the operation is the social worker group. The unit also includes a HVB home; Örtagården, outpatient care, our high-tolerance accommodation Stallgatan, and Housing First.

Some joint activities for residents are carried out, both inside the house and outside. Sometimes Örtagården's interventions are combined with psychosocial treatment in our outpatient care; or with counseling contact in primary care or psychiatry. The individual's needs determine the length of stay, which is why follow-up of the placement is important, as well as good discharge planning.

Together with the participant, you plan, execute, and follow up on the work, and you actively participate in the establishment and revision of the participant's implementation plan. Job tasks may include providing support in social contacts and leisure activities, accompanying to potential employment, assisting in contact with healthcare, and similar tasks. Practical support related to the participant's housing and daily living is also included. Collaboration with social workers is an important part, and you maintain contact with the participant even if they are cared for in an institution, serving a short prison sentence, or in inpatient treatment. Oral communication and written documentation requirements are part of the job.
