The Social Administration provides support to the residents of Botkyrka and is responsible for authority exercise, primarily according to the Social Services Act. The support is for both individuals and their relatives. The Social Administration has a special responsibility for children, young people, and adults in need of support according to social legislation. The Position and Responsibilities
We are looking for a Transition Coordinator for the Investigation of Young Adults who will strengthen the transition from placement or intervention to independent adult life.
In this role, you will be responsible for coordinating the work with young adults' transition from placement to increased independence. The target group consists of individuals who have been placed in, for example, community care, supported housing, training apartments, or foster homes, or who have participated in exit programs.
The assignment involves extensive collaboration, both locally and in the area where the individual will establish themselves. You will collaborate with the labor market and adult education administration, social services, property actors, employers, healthcare providers, and other relevant parties to create a sustainable transition to independent housing, work, or studies. In this role, you will work both operationally close to the individual and strategically to develop structures that provide continuity and quality.
Your main responsibilities include:
- coordinating transitions from placement to independence
- planning, coordinating, and following up on the individual's transition from external placement to independent living in collaboration with social workers and external actors
- mapping the individual's abilities, needs, and networks prior to the transition to independent housing, work, or studies
- activating and coordinating private and professional networks around the individual
- developing and ensuring quality in routines and collaboration forms for the work
- following up on the individual's establishment and contributing to the analysis of the long-term effects of the work.
The position is newly established and gives you the opportunity to help build a structured and cohesive function. The assignment involves travel within Sweden.

In the Investigation of Young Adults, 14 employees with broad and specialized expertise work. The group includes social workers, social workers focused on exits and crime, SIG/exiting coordination, case support, senior social workers, and unit managers. As a Transition Coordinator, you will receive present leadership, clear forums for case reviews, and a reasonable workload. You will receive a structured and individually tailored introduction as well as opportunities for collegial learning and joint reflection. Remote work is possible when tasks allow, such as during planning, documentation, and follow-up. We also apply annual working hours, which give you flexibility to plan your working hours based on the needs of the organization and your assignments. The assignment entails significant freedom under responsibility in how the work is planned and executed.
Who You Are
You have a degree in social work or another academic degree deemed equivalent. You have several years of experience working within social services and with the target group of young adults.
Other Requirements for the Position:
- Experience coordinating interventions between multiple actors.
- Experience documenting and following up on cases according to applicable legislation.
- Experience documenting, compiling, and following up on cases in Swedish.
- Experience working with the target group of young adults transitioning to independence.
- Driver's license (B) as travel is included in the assignment.
We consider it meritorious if you have:
- Experience planning and coordinating transitions from placement or supported housing to independent housing, work, or studies.
- Experience collaborating with labor market and educational actors.
- Experience in method or operational development within social work.
- Experience with Signs of Safety.
- Experience with the operational system Life Care.
As a Transition Coordinator, you are collaborative and relationship-building. The role requires you to build trusting collaborations with social workers, external providers, and establishing actors, as well as create a functioning dialogue with the individual and their network. By working openly and attentively, you can gather different perspectives and translate them into a common plan that supports the individual's steps towards independence.
You are structured and results-oriented. You plan, follow up, and ensure quality in each individual case while also identifying areas for improvement at a broader level. Through a methodical approach and the ability to analyze results, you contribute to sustainable routines and long-term strengthened quality in transition and establishment work.
