The Health and Care Administration, with approximately 1500 employees, is one of the administrations in Mölndal and includes elderly care, support for people with disabilities, and health care. The administration investigates and provides care and support to the residents of Mölndal according to the Social Services Act, the Act on Support and Service for Certain Disabled Persons, and the Health and Medical Services Act.

Team SAMSA is a planning unit with five social workers, one senior social worker, three planning nurses, and one unit manager, where we have close cooperation with the Bifrost short-term unit, and we share premises at Bifrost short-term unit. The main task is to be a strong partner in collaboration with inpatient care and primary care. The individual’s path through the discharge process should be as efficient as possible and the best for the individual. Social workers and planning nurses work closely together and often conduct planning meetings jointly.

RESPONSIBILITIES

You will investigate, assess, and follow up on decisions at the short-term unit. As a social worker with us, your main task according to the Social Services Act (SoL) is to investigate the need for support in connection with discharge from the hospital and plan for a safe return home based on the individual's needs.

You will perform regular authority duties according to the Social Services Act (SoL), where you investigate, assess, and follow up on decisions at the short-term unit. You analyze individual needs and then make well-founded decisions about appropriate interventions. The assignment includes collaborating with the region and primary care, as well as cooperating with other professionals to achieve a safe discharge.

QUALIFICATIONS

We are looking for someone with a degree in social work or another degree that can be assessed as equivalent by the employer. You have experience in authority duties and good knowledge of relevant legislation. It is considered an advantage if you are familiar with the procedures for admissions and discharges from inpatient health care and have knowledge of the IT services SAMSA, Treserva, Artvise Teams, and IBIC. A good digital understanding is required for the job.

We are looking for someone who is analytical and has the ability to identify the core of a problem, suggest possible solutions, and see connections. You use your time effectively and work systematically, organized, and methodically, ensuring that work is completed and followed up.

As a person, you show respect and consideration for those you meet in your professional role. You have an open and positive attitude and base your actions on the needs of the users. You easily adapt your working methods based on changing conditions and new information. It is important that you are independent and can act on your own and have the ability to make independent decisions within the framework of your assignment.

You have good knowledge of Swedish, both spoken and written, and have a B driver's license.
