The unit for palliative care and advanced home healthcare (ASIH) within Region Skåne has the overarching goal of providing equal care to all residents of Skåne regardless of diagnosis, age, or place of residence. In Kristianstad, the unit has a specific mission to provide palliative specialist care through home visits around the clock. The unit is divided into two parts that work closely together; the palliative care department and advanced home healthcare. In our palliative care department in Kristianstad, we have eleven care places, and within the municipalities of Kristianstad, Bromölla, and Östra Göinge, we have about 30 care places at home. We collaborate with municipal care and support to offer comprehensive care together with them.

The majority of our patients are cared for at home. Patients come to our palliative care department for shorter or longer stays, depending on the reason for admission. We also have a palliative consultation team that visits referred patients in hospitals and home environments and provides support to other caregivers. For the past year, we have also started a palliative specialist clinic (PM) during the day. This will, along with our consultative support, constitute a large part of our future mission where we actively work to meet the future of healthcare. This means that we will meet more patients at an early stage, and one step in this is Basonkology, which is a regional collaborative effort to bring care closer to patients. Individuals who have undergone, for example, cancer treatment are offered symptom relief and certain treatments at home.

Short decision-making paths give you great opportunities to influence your work. We have our own on-call line together with ASIH in Hässleholm, and the team's doctors are available around the clock.

RESPONSIBILITIES

With us, you will work with conversations aimed at supporting, encouraging, informing, and relieving patients and relatives in issues and problems related to the patient's illness. The tasks include assessing the psychosocial support needs of patients and relatives and providing information about societal resources. Additionally, you will hold group activities, such as survivor groups. You will work preventively, provide individual care, and help create security in difficult life situations. You will work with patients of all ages with various diagnoses.

QUALIFICATIONS

We now welcome you with a degree in social work and who is a licensed health and medical care counselor. Language skills in Swedish at least level C1 according to the Council of Europe’s scale are required. You have a genuine interest in palliative care and actively participate in the development and improvement work of the organization. The operation consists of, among other things, care departments, care places in the patient's own home or in special housing, which means that a valid B driver's license for a manual transmission car is required. It is also an advantage if you have completed step 1 of psychotherapy training and have previous experience in palliative care, oncology, or pediatric care and have worked with people in crisis and grief.

The work involves providing support and holding conversations, which requires you to be communicative and capable of meeting people at different levels, from various cultures and ages. You are skilled at building good relationships and enjoy collaborating with various stakeholders. You have a flexible approach and can prioritize between different tasks. The management of Close Care and Health has existed since January 1, 2025. The management is responsible for grant-funded care activities within close care. This includes, among other things, mobile teams and specialized palliative care, outpatient activities within close care, emergency care within close care, 1177 by phone, as well as certain region-wide functions. In addition, the management is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
