Welcome to apply for our group of social workers in specialized palliative care in Lund! We are currently four social workers, but one of us is retiring, and we are looking for a new colleague.
Palliative care and ASIH (Advanced Home Healthcare) in Lund is an operation tasked with providing palliative specialist care and consists of two units in close collaboration; inpatient care (Palliative Care Unit) and home healthcare (ASIH). The Palliative Care Unit in Lund has sixteen beds, and ASIH has about sixty home care places in eight municipalities in central Skåne, and we also have a palliative outpatient clinic.
ASIH in Lund is located in beautiful premises that we have designed ourselves for our home healthcare operation. We have doctors, nurses, social workers, physiotherapists, occupational therapists, nursing assistants, dietitians, pharmacists, and medical secretaries working with us. Our operation is characterized by continuous care development and education, where everyone is involved and responsible for the operation's goals, development, and results.
The majority of our patients are cared for at home, through ASIH in Lund, which performs specialist care interventions through home visits around the clock. Patients at our palliative outpatient clinic are also offered home visits. Patients come to the Palliative Care Unit for symptom relief optimization or for end-of-life care. We have a palliative consultation team that visits referred patients in hospitals and home environments and provides support to other healthcare providers. Collaboration often occurs with municipal care and social services to jointly offer comprehensive care.
JOB RESPONSIBILITIES
As a social worker with us, you will be responsible for social work tasks within the unit together with your three other social worker colleagues. Your work primarily involves conversations aimed at supporting, encouraging, informing, and relieving the patient and their relatives in matters and problems related to the patient's illness. Social work contact is offered to the patient and relatives, which takes place individually or in the form of couple and/or family conversations.
In the role of social worker, you will participate in medical rounds and lead psychosocial rounds. Job responsibilities include assessing and evaluating patients and relatives' needs for psychosocial support and counseling on issues such as relationship matters. The work also includes informing about community resources and answering questions related to finances, work, housing, transportation, insurance, and more. You will have access to continuous supervision, and if you are interested, there is also an opportunity to develop your knowledge within the profession.
QUALIFICATIONS
You who apply have a recognized degree in social work and several years of professional experience. We prefer that you are a licensed healthcare social worker. If you belong to a licensed profession, language skills in Swedish corresponding to at least level C1 according to the Council of Europe's scale are required. For non-licensed professions, good knowledge of Swedish in speech and writing is required. It is advantageous if you have previous experience in social work within palliative care and/or somatic care with patients of all ages and if you have psychotherapy training levels 1 and 2.
As a person, you have a genuine interest in people and focus on the patient and relatives. You enjoy working both independently and together with other professionals and have a good ability to prioritize among tasks. Furthermore, you are happy to share your knowledge and find it stimulating to work to optimize the quality of life for patients/relatives. You are secure in your professional role, engaged, and contribute to a good working climate. Additionally, you enjoy working in a large workplace with many colleagues. Great emphasis will be placed on personal suitability, where fundamental requirements are good social competence, good communication skills, and the ability to collaborate.

The Department of Close Care and Health has existed since January 1, 2025. The department is responsible for grant-funded healthcare activities within close care. This includes mobile teams and specialized palliative care, outpatient services within close care, emergency care within close care, 1177 by phone, and certain region-wide functions. Additionally, the department is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
