The Mobile Team for Primary Care Malmö/Trelleborg has now been operational for 1 year and is part of the Close Care in Malmö/Trelleborg. The healthcare providers involved include Sus Malmö, Trelleborg Hospital's contracted doctor car, ambulance services, private and public health centers, as well as other healthcare providers in the area such as the care and social services administration in the respective municipalities.
The Mobile Team for Primary Care is a cohesive form of care where primary care is the hub in collaboration with other providers. The team offers safe, clear, and accessible healthcare in the home, instead of hospital care, where the team can assess, plan, prescribe, and perform interventions. The care is specialized according to each individual patient's needs, and the goal is to offer patients qualified care both medically and psychosocially.
The team is based in Malmö and consists of doctors, nurses, social workers, and medical secretaries.

The role of the nurse involves independent work where you make your own home visits or team visits and provide healthcare in the patient's home. You lead and actively participate in planning around the patient's daily care together with other professionals and collaborate with other healthcare providers. You are also expected to actively participate in the development and improvement work of the organization.

Working hours are from 08:00 to 17:00, Monday - Sunday. Desired schedule and weekend duty every 5th weekend.
QUALIFICATIONS
We are looking for a licensed nurse with the authority to practice within Swedish healthcare, preferably with a few years of professional experience. Language skills in Swedish corresponding to at least level C1 according to the Council of Europe's level scale are a requirement. Since the operation consists of care placements in the patient's own home or in special housing, a valid B driver's license is a requirement for the position. We consider it an advantage if you have previously worked in primary care or municipal home healthcare, as we desire a breadth of competence and background in the team.
You who apply are secure in your professional role and have the patient in focus. Furthermore, you have good communication skills and find it easy to meet people on different levels. To thrive with us, you need to be able to work independently and make your own decisions while appreciating teamwork. The Department of Close Care and Health has existed since January 1, 2025. The department is responsible for grant-funded healthcare activities within close care. This includes, among other things, mobile teams and specialized palliative care, outpatient services within close care, emergency care within close care, 1177 by phone, and certain region-wide functions. In addition, the department is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
