Working as a Social Worker and Rehabilitation Coordinator at WeMind

The social worker acts as the link between healthcare and other societal actors, contributing the psychosocial perspective within psychiatry, alongside medical and psychological competence. Read more about the role of social worker/rehabilitation coordinator.

You will be part of an interdisciplinary team and contribute with knowledge about patient rights and opportunities for support in society. We have high ambitions for close collaboration within the clinic as well as with our patients, care partners, user organizations, and relatives. There are great opportunities for specialization within the profession.

Main responsibilities:
  • Assess the patient's social and economic situation and identify needs for support and interventions.
  • Coordinate interventions and collaborate with relevant actors such as care partners, social services, the Swedish Social Insurance Agency, and the Swedish Public Employment Service.
  • Convene and lead network meetings/SIP meetings.
  • Support sick-listed patients in the rehabilitation process and return to or entry into working life.
  • Collaborate with actors involved in the rehabilitation process.
  • Provide consultative support regarding social issues to other professions within the organization.
Your new workplace

WeMind Gröndal is a psychiatric outpatient clinic where we treat patients with psychotic illness, including investigations of new-onset psychotic illness. We also receive patients with diagnosed autism and low functioning level. You will be offered varied work where collaboration with other professions and care partners is a central part of your job.

We offer a structured workplace located in Gröndal with good transport connections. The unit features active and close leadership, short decision-making paths, and colleagues who collaborate and share experiences.

About you
  • Social work degree.
  • Experience from a similar role within psychiatry (meritorious).
  • Knowledge of the journal system TakeCare (meritorious).

As the hub of the network, it is important that you are flexible, professional, and have good collaboration skills. You have the ability to independently manage individual cases as well as plan and organize.

You have a strong commitment to your work with a high service level for patients who often have more severe mental disabilities. You contribute to a good work climate and value the social work environment and team spirit.

Practical information
  • Employment type: Fixed-term contract for 1 year.
  • Scope: Full-time 100% (70% social worker/30% rehabilitation coordinator).
  • Location: WeMind Gröndal.
  • Start date: As soon as possible, according to agreement.
