Coordinator/Case Manager for Adult Support Unit
Järfälla has a large open care system in constant development. The five open care units within Support and Treatment collaborate, plan, and develop together with high ambitions to make a difference for those we serve.
One of the units within Support and Treatment is the Adult Support Unit. Our unit currently consists of 21 engaged, knowledgeable, and unpretentious employees working with preventive, targeted, and outreach social work aimed at the adult population. The Adult Support Unit includes several different teams with various tasks, led by a unit manager and three team leaders.

We are now expanding our Housing First operations and are looking for someone who wants to be part of this exciting development work while making a difference for the most vulnerable, alongside a team of positive and competent colleagues. JOB RESPONSIBILITIES
As a coordinator/case manager within the Housing First coordinator team, you will work with the target group: adults who have experienced long-term homelessness often with dependency issues and/or other complex social problems, who will receive their own permanent housing without requirements for sobriety or other interventions. The work is based on the eight fundamental principles of Housing First, focusing on the client's autonomy and directing efforts towards harm reduction and recovery.
Your work is client-centered with the individual's needs in focus, and support is tailored individually based on the client's needs and wishes. A significant part of the work involves collaborating with others both internally and externally. In addition, the following tasks are included:
- responsibility for implementation plans and documentation in individual cases
- being part of the development work for the target group
- working to prevent evictions
- conducting group activities as needed
- working with individual conversations
In our work, we use Feedback Informed Treatment, FIT (ORS/SRS). FIT as a method helps us strengthen the individual's engagement and influence over their process, which can lead us to work correctly with our clients and give us the opportunity to continuously develop as therapists/coordinators.
Here, we work closely together, collaborate, and support each other when needed, making the work often enjoyable, varied, and stimulating. You currently have two colleagues and a team leader in your immediate work group. We have external supervision, continuous professional development, method training, close managerial support, and place great importance on a good and sustainable work environment. Some evening work will be included.
QUALIFICATIONS
We are looking for someone who is a social worker or has other education and/or experience that we assess as equivalent. Experience working with individuals in long-term, acute homelessness, with substance abuse/addiction and/or in vulnerable life situations is highly valued. Good knowledge of and preferably experience with the Housing First method is meritorious. A driver's license and driving experience are required as the work is variable and sometimes involves many relocations during the workday.
As a person, you are skilled at expressing yourself verbally and in writing and are clear in your communication. You need to be pedagogical and easily adaptable to new situations, as well as having experience meeting people with diverse life experiences. You need to be flexible in your approach. Furthermore, you have good self-awareness, are attentive, and unpretentious. You enjoy working independently and driving your own processes while we place great importance on you being a team player. We enjoy working together and collaborating with others.
Since the tasks are variable, you should enjoy challenges and be stimulated by flexible tasks. We place great importance on personal suitability and are looking for someone who can complement the group well.
Experience working to prevent evictions, with individuals in active addiction, or with untreated mental health issues is meritorious. MI training, training in low-affective approaches (LAB), and experience working with FIT (ORS/SRS) are also meritorious.
