Coordinator/Case Manager at the Adult Support Unit
Järfälla has a large open care system in constant development. The five open care units within Support and Treatment collaborate, plan, and develop together with high ambitions to make a difference for those we serve.
One of the units within Support and Treatment is the Adult Support Unit. Our unit currently consists of 21 dedicated, knowledgeable, and unpretentious employees engaged in preventive, targeted, and outreach social work aimed at adults. The Adult Support Unit includes several different teams with various tasks, and the work is led by a unit manager and three team leaders.

JOB RESPONSIBILITIES
As a coordinator/case manager within the coordinator team, you will work with adult individuals facing dependency issues and/or other complex social problems. You will support and coordinate towards an increased quality of life and a more independent life.
In your role, you will work with individuals who either live in our own boarding accommodation, in our training apartments, or in their own/other accommodations such as supported housing or HVB. Your mission is to ensure that the individual has coordinated and appropriate support to achieve change.
Your work is client-centered with a focus on the individual's needs. The support is tailored individually and always directed towards increased independence and quality of life. A significant part of the work involves collaborating with others both internally and externally. In addition, the following tasks are included:
- responsible for implementation plans and documentation in individual cases
- be part of the development work for the target group
- work preventively against evictions
- hold group activities when needed
- work with individual support conversations
- provide support in sobriety through conversations and drug and sobriety checks (supervised urine tests and breath tests)
In our work, we use Feedback Informed Treatment, FIT (ORS/SRS) and are currently participating in a research project in collaboration with Karolinska Institute. FIT is a method-neutral evidence-based quality assurance system that helps us strengthen the individual's engagement and influence over their process. FIT can guide us to work correctly with the right clients and provide us with the opportunity to continuously develop as coordinators/treatment providers.
At the Adult Support Unit, we work closely together, collaborate, and support each other when needed, making the work often both enjoyable, varied, and stimulating. You will have seven colleagues and a team leader in your immediate work group. We have external supervision, continuous professional development, method training, close management support, and place great emphasis on a good and sustainable work environment. Some evening work will be required.
QUALIFICATIONS
We are looking for someone who is a social worker or has other education and/or experience that we consider equivalent. Experience working with individuals with harmful use/dependency and/or in vulnerable life situations is highly valued. A driver's license and driving experience are required as work with clients is conducted in various locations and often involves many movements during the workday.
As a person, you are good at expressing yourself verbally and in writing and are clear in your communication. You need to be pedagogical and able to adapt to new situations easily, as well as have experience meeting people with diverse life experiences. You should be flexible in your approach. Furthermore, you have good self-awareness, are attentive, and unpretentious. You enjoy working independently and driving your own processes while we place great importance on you being a team player. We enjoy working together and collaborating with others.
Since the tasks are changing, you should like challenges and be motivated to work flexibly. Experience working preventively against evictions, with various housing solutions, or with individuals with different types of functional variations is meritorious. MI training and experience working with FIT (ORS/SRS) are also advantageous.
