Social Welfare Officer with Responsibility for SVU

We are now looking for a social welfare officer responsible for SVU to join an experienced team where collaboration, quality, and commitment permeate daily work. Assignment

In connection with a retirement, we are looking forward to welcoming a new colleague to the workgroup. Here you will play a key role in our team responsible for safe transitions between hospitals and home. The role consists of a meaningful assignment where your competence and commitment make a difference for people.

As a social welfare officer responsible for SVU, Collaboration upon discharge from inpatient care, you have a central role in planning and coordinating efforts in a safe, professional, and trustworthy manner. The assignment also involves investigating, making decisions, and following up on efforts according to the Social Services Act.

In this role, you will work closely with both inpatient and primary care via the IT support Mina planer and be part of the municipality's SVU team together with licensed personnel. Furthermore, you will collaborate closely with the municipality's home team to enable safe discharges. Planning and coordination of efforts will be done in collaboration primarily with colleagues in home care, home health care, and short-term accommodation.

Competence

We are looking for someone who is a trained social worker or has equivalent education and experience that the employer assesses as equivalent. You have previous experience working as a social welfare officer and good knowledge of relevant legislation.

It is advantageous if you have experience of authority exercise in the SVU process and if you have worked in the operational systems Lifecare Handläggare and Mina Planer.

To succeed in this role, you are structured, independent, and solution-oriented. You possess a good ability to collaborate and coordinate efforts where you ensure that the right information reaches the right people through clear communication.

Furthermore, you are flexible and can prioritize in a job that is sometimes characterized by a high pace. You express yourself well in both speech and writing, enjoy phone contacts, and feel confident in the role of a case officer.

A driver's license (B) is a requirement.

Working Hours/Duration

Permanent full-time employment. Start date according to agreement.
