Responsibilities

As a Coordinator, you work with varied tasks where, together with other coordinators, you plan and administer the daily operations for both day and evening activities. You work towards good quality, balanced finances, and a safe working environment. The role involves working according to the home care quality assurance process in accordance with the Social Services Act, following planning processes, and documenting in LifeCare and Mina Planer to ensure good collaboration when receiving new customers and patients returning home from the hospital. You also collaborate with customers, relatives, authorities, and other partners based on the needs of the operations.

You collaborate closely with the unit manager, administrator, and your coordinator colleagues regarding planning, customer contact, scheduling, staffing, and quality work. You also contribute to follow-up and analysis of operational needs and actively participate in various meeting forums. Part of the role involves planning and coordinating the introduction of new employees.

You also work practically out in home care and, as needed, carry out both planned and unplanned care interventions for customers.

Qualifications

We are looking for you who are trained as a nursing assistant and have a certificate of protected professional title from the National Board of Health and Welfare, or can certify that you had a permanent position as a nursing assistant on July 1, 2023. You are comfortable working with various digital tools and are confident in your documentation skills. You need to have basic computer skills. Furthermore, you must be willing to be delegated and perform delegated interventions according to the Health and Medical Services Act, have good knowledge of Swedish (both spoken and written), and hold a B driving license.

It is meritorious if you have previously worked with LifeCare (LCU, LCP, and LMO), Mina Planer, and Medvind. Experience in similar work is also considered meritorious.

Working Hours: You will work on a schedule for both day and evening shifts. The position includes weekend work.
