About the Workplace

At the turn of the year, we started our new unit Adults and Disability. Here, service delivery and authority functions are combined. The unit covers housing support and employment activities, as well as case processing within social psychiatry, harmful use and addiction, the Act concerning Support and Service for Persons with Certain Functional Impairments (LSS), violence in close relationships, and support for individuals wishing to leave a criminal lifestyle.

We are developing working methods with a focus on flexibility, accessibility, and individual adaptation. This means we need to be able to adapt quickly to changing needs and offer the right support at the right time. Among other things, we aim to eventually offer more family-oriented housing support based on the PYC method. We also see a need to work more proactively within all our target groups to identify individuals before problems become severe.

In this role, you work flexibly, where the work can alternate between housing support and proactive outreach – depending on what best meets the individual's needs and the organization's priorities.

Job Responsibilities

As an employee within social flexible support, you work closely with people who have varying needs for support in their daily lives. Your work may involve interventions both within housing support and proactive outreach, and you need to be prepared to switch between these tasks.

In the role of housing support, you support individuals in their home environment with, for example:

  • Structure in daily life.
  • Practical chores.
  • Contact with authorities and healthcare.
  • Support in creating and maintaining functional routines.
  • Structure in the daily life of the family and children.

In the role of proactive outreach, tasks include, for example:

  • Informing about support and opportunities in society.
  • Guiding individuals to the right support and interventions.
  • Promoting health, participation, and social community.
  • Collaborating with various actors to create the best possible support.
  • Planning and implementing information and dialogue initiatives.
  • Developing networks and forms of collaboration.
  • Contributing to the development of the municipality's preventive work.

Common to the assignment is that you:

  • Work relationally and motivationally.
  • Adapt the support based on the individual's daily condition and needs.
  • Collaborate with colleagues and other actors regarding the individual.
  • Contribute to creating continuity and security in the support.

The work is varied and requires that you can quickly switch between different tasks, target groups, and working methods.

Requirements:

  • A B-class driver's license is required.
  • Good computer skills and experience with documentation are required.
  • Ability to work in homes with animals is required.

Qualifications

  • Upper secondary education in healthcare and social care, social work, or other relevant education (post-secondary education is meritorious).
  • Experience working with adults within social psychiatry and disability services.
  • Experience in providing supportive and motivational work.
  • Good communicative skills in Swedish.
  • Good computer skills and experience with documentation.

It is meritorious if you have:

  • Experience with housing support or proactive outreach.
  • Experience with co-morbidity and complex problems.
  • Experience with low-affective handling or motivational interviewing (MI).

Employment Terms

Fixed-term position with the possibility of extension.

Full-time.

Start date: By agreement.
