We are now looking for an engaged Home Care Coordinator for the Centrum unit!

The position is a temporary assignment, initially for 6 months, but it may become longer.

The Care and Elderly Services Department in Borås Municipality works towards the goal that both younger and older residents in need of care, support, and services, as well as healthcare and preventive health work, should have a good life every day.

The department consists of approximately 2,500 employees, of whom approximately 700 work within the municipal home care service.

About the operations

We who work within Home Care Centrum are a wonderful team consisting of assistant nurses, care assistants, service assistants, managers, coordinators, and home care coordinators. Men and women with different backgrounds work here, which enriches our operations and broadens our competence.

Home Care Centrum is based in spacious and pleasant premises on Engelbrektsgatan 2 in Borås.

Centrum Home Care is a well-functioning operation characterized by commitment, competence, and good cooperation. What distinguishes the unit is that many employees have worked at this unit for several years, and that the employees have long and broad experience in care and support. Here we help and support each other; there is a strong team spirit where all professions work together to create the best possible conditions for our users.

Responsibilities

Working within home care is about doing something important for real. As a coordinator, you will have a central role in the operations with varying tasks. You will become an important part of an experienced team where you have the opportunity to influence, contribute to development, and make a difference every day.

As a home care coordinator, your main task is to plan and schedule ordered interventions from assistance handlers and healthcare personnel. You work towards set goals and strategies and have close cooperation with the unit managers in home care as well as several other professional groups. Your task is to optimize the scheduling in a resource-efficient way, which among other things means finding logical solutions for intervention planning with the user in focus.

You have daily communication with SÄS (Social Services) where you monitor users' discharge and return home.

The Home Care Coordinator is also responsible for handling staffing during short-term absence and, if necessary, rescheduling interventions.

The Home Care Coordinator works in systems such as TES, SAMSA, Time Care pool, VIVA, etc.

Daily use of Excel, Word, as well as communication via email and phone.

Working hours are during daytime on weekdays and occasional weekend shifts.

Qualifications

You should have post-secondary education in administration or at least one year of documented experience in planning home care interventions. If you have worked as an assistant nurse within home care, this is an advantage.

We are looking for you who have good analytical as well as logistical skills. You have commitment and a desire to develop and improve planning based on set goals within the Care and Elderly Services Department. You should have good communication skills both verbally and in writing, as well as good teamwork skills and a high sense of service.

The work may involve rapid changes and requires flexibility in your working method, where you should be able to handle several tasks simultaneously. You must also have good computer skills and the ability to handle different operational systems and the Office package.

You need to understand the relationship between the different parts that govern home care, i.e., orders based on the Social Services Act (SOL) and the Health and Medical Services Act (HSL). You also need to have an understanding of the relationship between the unit's needs, personnel, and economy.

It is important that you are professional and can make decisions based on your role of responsibility.

Great importance will be attached to personal suitability.

A B-driver's license is required for the position.
