Hospital at Home is run by the Business Area (VO) Emergency Care and Internal Medicine in collaboration with the Business Area for Infectious Diseases. This means that patients in need of acute specialist care can be offered this in their own home instead of on a hospital ward.

The inspiration for this care model originally came from other countries where the concept of "Hospital at home" has been used successfully. Studies show several positive effects for patients cared for at home, such as better eating and sleeping, increased activity, and a reduced risk of acute confusion or falls. Experiences so far show very high patient satisfaction and patient participation.

RESPONSIBILITIES

We are now looking for you, a nurse, who wants to be part of Hospital at Home Lund.

Instead of the patient being admitted to a hospital bed, we as healthcare professionals come to the patient's home to provide the care that is currently performed in inpatient wards. Patients can be monitored remotely via digital tools and can always quickly contact us at Hospital at Home and be admitted to the hospital if their condition requires it.

We strive for and continue to maintain broad competence within the team, which we value highly, as our patients can come from all different units within the hospital, although most are treated for infections requiring antibiotics. We constantly strive to develop and actively work on which patients we can care for and how we can include and take care of as many patients as possible in the best way possible.

The role primarily involves independent work with patients in their homes, where we perform nursing treatments and assessments, as well as medical treatments and assessments. We can handle much of the medical interventions that can be performed on a regular hospital ward. Our patients come directly from the emergency department, from hospital wards, and directly from primary care. It may also involve treatments at the hospital for our patients enrolled in Hospital at Home. Some digital work is also included, such as video rounds with patients and coordination of patients within the hospital.

Patients live around central Skåne, so you will travel by car and bicycle. Working hours are scheduled for day and evening shifts, both weekdays and weekends, without night duty.

The positions are full-time, but part-time opportunities may be offered if interested.

QUALIFICATIONS

We are looking for you who are a licensed nurse with the eligibility to work in Swedish healthcare. Proficiency in Swedish at least corresponding to level C1 according to the Council of Europe's scale is a requirement. For the work, you must have previous experience as a nurse and a valid B driver's license. If you have specialist training in infection, medicine, or similar, it is meritorious. Likewise, if you have experience in internal medicine, infectious disease care, other inpatient care, and/or municipal health and medical care.
