Process Leader for FACT – Integrated Care for Adults
Make a difference. Every day.
Do you have a strong interest in leading projects and are looking for a new challenge? Then this might be the job for you!
The Business Area (VO) Adult Psychiatry Kristianstad provides outpatient care in Hässleholm, Kristianstad, Simrishamn, and Ystad. The emergency clinic and all inpatient care are located in Kristianstad. For residents of Ystad, Skurup, and Sjöbo, inpatient care and emergency services are offered in Malmö and Lund. Our catchment area covers the north and south-eastern municipalities of Scania, with a total population of approximately 300,000 people. We have a good collaboration with the municipalities through cooperation meetings and care planning. Despite a large geographical spread, our operations are characterized by proximity and short decision-making paths.
As part of the collaboration on mental health and suicide prevention, a development project is underway between the region and five municipalities in south-eastern Scania. By introducing integrated working methods, healthcare and social care interventions can be provided in a more person-centered and cohesive manner, ensuring that individuals with complex needs receive continuity, holistic care, and participation in their treatment. The model on which this work is based is Flexible Assertive Community Treatment (FACT), and we are currently in an exploratory phase. To drive this work forward, we are seeking a Process Leader for FACT with a focus on adults as the target group. The work is carried out in collaboration between adult psychiatry in Ystad and Simrishamn, as well as the five municipalities in south-eastern Scania: Simrishamn, Sjöbo, Skurup, Tomelilla, and Ystad. The work on introducing integrated working methods is also ongoing in parallel for children and young people, where a separate Process Leader for FACT has now been appointed.

RESPONSIBILITIES
In this role, you lead and coordinate the work of implementing the FACT model, in close collaboration with both the municipality and the region. You are employed by Region Skåne but have a cooperation assignment stemming from the entire psychiatric collaboration in south-eastern Scania, which means you also represent the five municipalities. Your duties include developing common routines and guidelines, as well as any agreements for the FACT collaboration. Additionally, you plan for and/or participate in training and inform about FACT in the sub-region.
You work closely with the process leader who has a corresponding role for the development work regarding children and young people in south-eastern Scania, as well as with the sub-regional coordinator for mental health and suicide prevention. There is established collaboration where you will be a coordinating part of a consultation group with broad competence and high engagement working on these issues. You have a driving and supportive role in the work of exploring, and upon decision, implementing integrated working methods through FACT. The assignment involves creating conditions for joint stances and leading the process forward in a balanced and transparent manner.
The position is an initial project appointment for 1 year, with the possibility of extension. The employment rate is part-time. The main place of work is Ystad, but work towards the entire south-eastern Scania is part of the role.
QUALIFICATIONS
We are looking for you who have a approved healthcare education or another university education that we assess as relevant for the position. If you belong to a licensed profession, Swedish language skills corresponding to at least level C1 according to the Council of Europe scale are required. For non-licensed professions, good knowledge of Swedish in speech and writing is required. Experience of cooperation work between region and municipality is meritorious. As a person, you are structured and accustomed to working independently. Additionally, you are flexible and enjoy both cooperating and collaborating with others. You have good communication skills, are receptive, and can meet people in different roles and contexts. Furthermore, you work from a holistic perspective in your various processes. We place great emphasis on personal suitability.
