We offer an attractive workplace that gives you great opportunities to develop in your professional role. Our goal is for everyone to thrive and grow throughout their entire career. We also want to develop as an employer and actively work to make the best use of your knowledge and commitment. We appreciate and encourage participation from everyone.
Uddevalla Municipality is a beautiful location by the sea in the heart of Bohuslän. We believe in an open and positive interaction between the municipality, education, business, and culture.
We hope you will enjoy working with us!
About the job
Do you want to help shape tomorrow's coordinated care and social services? We are expanding our staff and are now looking for you who want to take on the role of welfare advisor at the emergency department of Norra Älvsborg County Hospital (NÄL) – a position where the individual's security, independence, and opportunity for support are at the center.
As a welfare advisor, you will work in a unique one-year project covering Uddevalla, Vänersborg, and Trollhättan. Your task is to meet residents at the emergency department who do not require inpatient care but may need support at home. Through counseling, coordination, and decision-making, you create conditions for a safe and secure return home from the emergency department and reduced readmissions.
You have the delegation to make decisions according to the Social Services Act and work closely with various professions within regional and municipal operations. The assignment is part of the transition to the new Social Services Act and the national transition to close-to-home care, which is based on collaboration, innovation, and individualized solutions.
During the project period, Uddevalla Municipality is the employer for the role of welfare advisor. You will share the position with a colleague, meaning you will work 50% as a welfare advisor with your workplace at Norra Älvsborg County Hospital (NÄL) and 50% as a benefits caseworker in the planning team at the benefits unit in Uddevalla Municipality.
Responsibilities
- Meet individuals at the emergency department who need counseling and support.
- Conduct advisory conversations and motivate independence.
- Receive applications, investigate, and make decisions on interventions according to the Social Services Act.
- Collaborate with healthcare staff and municipal actors.
- Ensure information transfer and documentation.
- Conduct follow-up conversations after discharge home if necessary.
- Inform about municipal and civil society resources.
You meet residents who, according to licensed healthcare personnel, do not need inpatient care – both those with and without previous contact with social services. Special focus is placed on the elderly and people who risk losing their independence without the right support.
We are looking for you who
- Are curious, interested in working preventively, focus on finding solutions, and have a flexible working style.
- Have knowledge of the Social Services Act and welfare technology.
- Are confident and proactive in a new, unknown environment.
- Are confident in making decisions and working independently.
- Have good cooperation skills and understanding of the different roles in healthcare and social care.
Great importance will be placed on your personality and that you are the right person for the role of welfare advisor.
Qualifications
- Degree in Social Work (Social Worker).
- Category B driver's license.
- Experience working as a benefits caseworker.
Other
Your employment will fundamentally be as a benefits caseworker within the Adult Support department in Uddevalla Municipality, but during the project, you will work as a welfare advisor/benefits caseworker as described above.
The assignment is monitored through measurable indicators such as the number of conversations, decided interventions, and readmissions. Your contribution makes a difference – both for the individual and for the efficiency of care.
