Bellevue Health Centre operates on behalf of Region Skåne. The centre has just over 4,000 registered patients, and is staffed by doctors, nurses, assistant nurses, psychologists, physiotherapists, and more.

Job Description:

We are looking for a social worker (counselor) who will also work as a rehabilitation coordinator at Bellevue Health Centre and Hälsan Malmö Health Centre. In this role, you will have overall responsibility for the rehabilitation coordination of patients.

The position involves working with primary care tasks, assessing and treating patients with mild to moderate mental health issues, including internet-based Cognitive Behavioural Therapy (iCBT), as well as coordinating and following up on rehabilitation efforts for patients needing support in their recovery after illness or injury.

As a social worker, you will meet patients with common issues such as depression, stress-related ill health, anxiety, and sleep difficulties. You will conduct assessments, initiate treatments, and hold initial contacts with patients. If necessary, you will coordinate further interventions within the care team. Together with the centre's doctors, you will participate in joint rounds where patients are discussed.

In the role of rehabilitation coordinator, you have overall responsibility for planning, coordinating, and following up on rehabilitation efforts for patients with various needs. The work includes, among other things, creating rehabilitation plans, collaborating with physiotherapists and other healthcare staff, and supporting patients in planning their return to work or other activities. You serve as a crucial link in the care chain, ensuring that patients receive cohesive support and continuous follow-up throughout the entire rehabilitation process.

Requirements for the role:

• Degree in Social Work and training as a rehabilitation coordinator.
• Training to conduct iCBT (internet-based Cognitive Behavioural Therapy) treatments.
• You will work across the entire group, coordinating rehabilitation efforts for patients registered at all four of our health centres.
