JOB RESPONSIBILITIES

With us, we work cross-professionally. As a nurse in short-term care and the Reception Team, you will collaborate with occupational therapists, physiotherapists, nursing assistants, and unit managers. The work also involves collaboration with case managers. You will work to provide the patient with opportunities for recovery, training, and rehabilitation to return home from short-term care or directly to their home environment for a period of about 2-3 weeks.

As a nurse in short-term care and the Reception Team, you will have a varied job where each day can look different. You will have the opportunity to work in the short-term care unit as well as in home environments. As a nurse, you will closely collaborate with Region Skåne in planning for discharge. This takes place in the shared IT system Mina planer, where all planning for cooperation during discharge occurs. For the nurse, the work largely involves collaboration with the region, primary care, and relatives.

Traditional tasks are also included for a nursing responsible nurse within municipal health and medical care. Guidance and delegation of nursing staff are also part of the job. The focus of the work is on teamwork and thus creating opportunities for our patients to feel safe and involved in planning.

QUALIFICATIONS

We are looking for a licensed nurse with good knowledge of municipal health and medical care. As a nursing responsible nurse, you are part of maintaining patient safety in Landskrona Omsorg. We prefer that you have experience in the profession; it is a merit if you have further education in geriatrics or similar.

Since communication and documentation are part of the job responsibilities, you need to have good computer skills and be proficient in Swedish, both spoken and written. We would like you to have good experience documenting in the Lifecare journal system and experience working in the Mina planer and Pascal systems. A B driver's license is required for the position.
