We are now looking for social workers for the planning organization’s units in Centrum, Hisingen, and Nordost.

The positions are located at Vågmästaregatan 1E.

The mission is to be a natural strong partner in collaboration with inpatient care and primary care. The individual’s path through the discharge process should be as efficient as possible, with the individual’s best interests always in focus.

The future requires different working methods, and therefore the planning organization needs to be represented in joint forums. We raise the issues where our professional competence has much to contribute so that the care that must complement the healthcare becomes visible. Together, we develop the work in harmony with the guidelines and agreements that exist for the area.

Main tasks of the planning unit:

• The main task is, in accordance with SOL, to investigate the need for support in connection with discharge from the hospital and to plan for a safe return home.
• Collaborate with the region, primary care, and municipal primary care in connection with admissions and discharges based on the individual’s needs.
• Follow up on short-term care. The planning unit follows up on short-term decisions until the person leaves short-term care or is granted a care and nursing home.
• Active collaboration with other professionals to achieve a safe return home.

About You

Requirements

• Degree in social work.

Meritorious

• Experience in public authority exercise.
• Familiar with the routine for admissions and discharges from inpatient healthcare.
• Knowledge of the IT services SAMSA and Treserva.

You are friendly and helpful to others even in stressful situations and show respect for everyone you meet at work. You can quickly change your perspective and approach, see opportunities in changes, and are attentive and open to understanding other people's needs, feelings, and opinions.
