Specialist Physician for Mobile Team Primary Care in Ängelholm

At Mobile Team Primary Care (MTN) in Ängelholm, our days are shaped by new assignments where each patient encounter is unique. We offer safe, clear, and accessible care for patients who need and wish for healthcare at home instead of in the hospital.

Patients receive prompt medical assessment and treatment at home that would otherwise be performed at a health center or hospital. In cases where patients may need hospital care, we strive for direct admissions to the ward.

As a physician with us, you will closely collaborate with the rest of the team and other healthcare providers. The team consists of 12 members, including physicians, nurses, social workers, and medical secretaries. Care is provided in collaboration with home healthcare, health centers, hospitals, and ambulances, always with the patient at the center.

We operate every day of the year from 8:00 AM to 5:00 PM, which means that some weekend work is included in the position. The team is based in facilities at Ängelholm Hospital and visits patients in six northwestern municipalities.

RESPONSIBILITIES

With us, you will be offered a developing and exciting work environment where you, together with us, can provide patients with qualified care, both medically and psychosocially. In your role, you will perform medical assessments and interventions directly in the patient's home or in municipal housing. You will lead and actively participate in planning the patient's care in close collaboration with other professionals and healthcare providers.

QUALIFICATIONS

We are looking for a licensed physician with the authority to practice within Swedish healthcare and have specialist competence in geriatrics, internal medicine, general medicine, or other relevant specialties. Knowledge of the Swedish language at least at level C1 according to the Council of Europe's scale is a requirement for the position. A valid B driver's license is preferred, as care is primarily provided in patients' homes.

As a person, you have good professional skills, are confident in your role, and have a patient-centered focus with the ability to see the individual needs of patients. Part of the work involves support and conversations, which means you need to have strong communication skills to meet people at different stages of life. With a flexible approach and an empathetic and attentive demeanor, you create safety and trust in every patient encounter. Great emphasis is placed on good collaboration skills and personal suitability.

This includes mobile teams and specialized palliative care, outpatient services within close care, acute care within close care, 1177 by phone, and certain regional functions.
