The Mobile Team for Primary Care in Ystad continues to grow – and we are now looking for an experienced and confident nurse who wants to help develop the team together with us. We are receiving more and more assignments and need to strengthen the team with another colleague who is passionate about person-centered care and wants to work with home visits to patients.

The Mobile Team for Primary Care is part of close care and works to ensure that patients can receive safe, secure, and high-quality care in their own homes instead of in hospitals. The team consists of nurses, doctors, and social workers who work closely together. We have a close collaboration with health centers, home care, ambulances, and hospitals. The work is characterized by cooperation, flexibility, and care, with the patient's needs as a natural starting point.

RESPONSIBILITIES

We now welcome a nurse to the Mobile Team for Primary Care in Ystad.

The role of the nurse involves independent work where you conduct your own home visits or visits in teams. You actively participate in planning the patient's care together with the team and collaborate with other healthcare providers/partners. Your tasks consist of providing medical care at home, which may include intravenous antibiotic treatment, intravenous fluid treatment, and blood transfusions. Participation in the development and improvement work of the organization is also included in the tasks.

QUALIFICATIONS

We welcome you who are a licensed nurse with permission to practice within Swedish healthcare. Additionally, you have several years of experience working in somatic care. Language proficiency in Swedish at least at level C1 according to the Council of Europe's scale is a requirement. Since we provide care in the patient's home or in special housing, a valid B driver's license is also required.
