JOB DESCRIPTION

Are you a registered nurse looking for a varied and developing job?

Do you want to ensure that patients feel safe and supported in their own homes after a hospital stay?

As a nurse in the reception team, you will work with the discharge process of patients from the hospital to their own homes. You will be an important part of the discharge process and contribute to creating safety and independence for our patients. You will have a varied role where you work both in home environments and in short-term care units. Together, we work to provide patients with opportunities for recovery, training, and rehabilitation to return home from short-term care or directly to their home environment for a period of about 2-3 weeks.

Your tasks include:

• Responsible for care and medical interventions within municipal health and medical care.
• Participate in the discharge process from hospital to home via short-term care, where you also support and motivate patients, relatives, and staff.
• Collaborate with Region Skåne, primary care, relatives, and internal teams, as well as supervise and delegate to care staff.
• Document in journal systems and planning tools.
• Work cross-professionally with occupational therapists, physiotherapists, nursing assistants, and case managers.

QUALIFICATIONS

We are looking for you who are a registered nurse and want to work with the discharge process and short-term care!

You have:

• Registration as a nurse.
• Good knowledge of municipal health and medical care.
• Good computer skills and proficiency in Swedish, both spoken and written.
• Driver's license B.

You may have experience in:

• Municipal health and medical care.
• Further education in geriatrics or another relevant area.
• The journal system Lifecare and planning tools Mina planer and Pascal.
