Job Responsibilities
We are looking for two specially qualified contact persons starting April 1st. As SKKP, you will work closely with individuals who may face challenges related to, for example, substance abuse, crime, school absenteeism, mental health issues, and/or NPF. The assignment involves being a stable adult support outside the home, focusing on change, motivation, and long-term development.
The support is tailored individually based on an implementation plan and takes place in close collaboration with social workers. The work is relationship-based and can encompass up to 10 hours per week for about six months, depending on the youth's needs.
In the assignment, you create a safe and trusting relationship and provide stable emotional support in the individual's daily life, both in good times and bad. You provide individualized support focusing on motivating school attendance, work, and meaningful leisure activities, as well as helping to create structure, break isolation, and establish functioning routines.
You also provide practical support based on needs, such as homework assistance or contacts with schools, authorities, and healthcare. The work is carried out through conversations and motivational interventions, in close collaboration with the individual's network and internal and external actors such as social services, correctional services, police, and healthcare providers. The assignment is documented according to applicable legal requirements and is carried out on behalf of social workers according to SoL and LVU. The goal is to contribute to sustainable changes that persist even after the intervention has ended.
About the Workplace
The main mission of the Social Administration is to offer individualized support and services in elderly care, disability, children and families, adults, and financial assistance. We are present for the citizens of the municipality throughout all stages of life.
On July 1, 2025, the new Social Services Act came into force, which means that the work of social services needs to be adapted and improved. The Social Administration in Nynäshamn Municipality is working to ensure that we have the right functions and competencies to offer the best possible support. The support should be based on the individual's needs and grounded in evidence and knowledge according to the intentions of the new law. Promotion, prevention, and outpatient care are gathered under a common department, the department for social support. The position of specially qualified contact person (SSKP) falls under the outpatient care unit. Our goal is to work with more promotional and preventive measures now that the new law is in place!
