As a case officer, you will actively work with social work in line with the intentions of the new Social Services Act – to promote health, strengthen individual capabilities, and work preventively. You see support measures as something that should promote independence, and you meet each person with a focus on resources, participation, and development.

As a case officer, you are a central part of this success – you lay the foundation for our efforts to be both understandable and meaningful.

You collaborate closely with the municipality's service providers such as home care, special accommodations, and family support. If necessary, you also cooperate with the municipality's health and medical care, for example, nurses, occupational therapists, and physiotherapists. When needed, cooperation also occurs with the region's health and medical care, rehabilitation, primary care, and other actors to create a secure whole for the individual.

We use modern systems and working methods to support case management – to free up time for what is most important: the meeting with the individual.

JOB RESPONSIBILITIES

As a case officer, you will work with investigation, assessment, decision-making, and follow-up according to the Social Services Act (SoL) within elder care.

Your main tasks are to:

  • Investigate and assess the need for support and measures
  • Make decisions about measures according to SoL
  • Follow up on decisions and ensure that the measures work as intended
  • Have dialogue and collaborate with care recipients, relatives, and guardians

The work is based on the Individual's Needs in Focus (IBIC), which provides structure and clarity in the needs assessment.

QUALIFICATIONS

We are looking for someone who is a social worker or has another equivalent higher education. You are confident in case management according to SoL and have a good ability to express yourself in Swedish both orally and in writing.

Experience in case management within elder care is an advantage.

Other merits:

  • Experience with IBIC
  • Experience in collaboration with municipal care services or health and medical care

You are accustomed to modern working methods and see technology as a natural support. We place great emphasis on personal qualities such as responsiveness, relationship-building, and the ability to contribute to a good working environment.
