We offer you a shared position between the geriatric clinic's outpatient services, the geriatric advisory team, and some duties in the geriatric ward as agreed.

Most patients come to our geriatric outpatient clinic for dementia assessments. Here we work in teams, but you also have your own clinic to perform dementia tests. The work includes, among other things, telephone advice to patients and relatives as well as medication follow-up. Work with medication studies is also included.

You will also work in our geriatric advisory team where you are part of a team together with doctors, medical secretaries, pharmacists, and occupational therapists. The team is based at the County Hospital's emergency department and assesses the most fragile elderly patients who come to the emergency room. The team decides whether the patient can return home or needs to come to our acute geriatric care ward. We collaborate closely with primary care and the municipality. The geriatric advisory team also provides advice and support to other caregivers regarding the most fragile elderly group. The goal is to create security for this group, wherever they are, by working in an advisory and supportive manner.

In the ward, you are responsible for ensuring care and following up on medical prescriptions.

We work based on person-centered care, which is an ethical approach that involves seeing the individual person. It is therefore important to involve and adapt to the individual's needs, resources, and circumstances. The starting point in person-centered care is the patient's narrative about their illness, sometimes with the help of relatives. Based on the story, a joint care plan is made with goals and strategies during the care period. Participation is a fundamental aspect of person-centered care and means, among other things, that there is room for the care seeker to express questions and any concerns.

At the clinic, we offer two university courses in geriatrics and specialized palliative care, each worth 7.5 credits.

Interviews are conducted on an ongoing basis, and hiring may occur before the application period ends.

About you

We are looking for someone who wants to help develop care for the most fragile elderly at the geriatric clinic. You are a licensed nurse, preferably with further education in geriatrics/dementia and experience from the municipality. You have a genuine interest in meeting elderly patients with severe illnesses. Teamwork is important to us, and you are flexible and good at collaborating. We value that you share our vision of good treatment and participation. You are outgoing, responsible, service-oriented, and confident in your role, with the ability to prioritize. Great emphasis will be placed on personal suitability.
