In collaboration between primary care, specialist care, and municipal health care, we have established a Home Care Team. The focus is on ensuring that patients in Västerås and Surahammar receive safe and professional care in their homes.

Responsibilities

Working as a nurse with us is a varied job where no two days are alike. You will make assessments and visits in patients' homes, often in teams with doctors, other nurses, or nursing assistants, but independent visits also occur. Collaboration with home health care and other care providers is central to the work. The nurse in the home care team often acts as a coordinator and cohesive link to ensure optimal care around the patient. SIP is conducted for most patients. Standard nursing tasks are included. If you have experience and feel confident handling medical technical tasks such as central lines, it is an asset. Tasks such as intravenous infusions and blood transfusions are part of the role.

About the Workplace

The Home Care Team in Västerås was established as a project in the summer of 2022. From January 1, 2024, we will be a regular operation aimed at complementing the care structures that previously existed.

Home care means that we are positioned between primary care clinics and specialist care at the hospital. Our goal is to serve as the cog needed for complex and severely ill patients in Västerås and Surahammar municipalities to have their care needs optimally met at home. We exist for those who have difficulty accessing care, and our mission is to create security at home and avoid hospital admissions.

The team is based in facilities at Ängsgärdet and consists of doctors, nurses, nursing assistants, and social workers.

Qualifications

We are looking for a licensed nurse with at least five years of experience in the profession. If you are a trained district nurse and/or have experience caring for patients at home, we consider it an asset. Since you will need to communicate verbally and in writing with patients, relatives, and colleagues, good knowledge of the Swedish language is required. A B driver's license is a requirement for the position.

As a person, you are confident and stable in your profession, empathetic, and have an easy time collaborating with others. You are well-structured in your work, take responsibility for your tasks, and contribute to a positive work environment. Since we care for patients in their homes, it is also important that you are flexible and can adapt according to the person and situation. We place great emphasis on personal suitability.

Employment Conditions

Permanent, full-time, daytime Monday to Friday. Start date by agreement.
