Job Responsibilities

You will have your own diabetes clinic and will also perform standard nursing tasks together with your colleagues. The role of a diabetes nurse means being able to collaborate in a team with several of the health center's professions and functions to ensure good care, excellent service, and the best possible health for the individual. We are a close-knit workgroup where we help and support each other. We also have a coaching, developing, and present leadership to support us. Improvement work for the entire health center is an important part of our daily work.

About the Workplace

The 11 region-run health centers in Västmanland operate as an intrapreneurship with a civil servant board and Region Västmanland as the owner. We share a common name, Närvården Västmanland, and work together to provide our patients with the best primary care in the region. We continuously invest in the development of our workplaces to meet the future needs of primary care.

Närvården Bäckby is a growing health center, and we have a branch in Råby. We are responsible for over 11,200 registered patients, and our unit includes medical and district nurse clinics, various types of specialist clinics, maternal and child health care, as well as a rehabilitation unit with physiotherapists and a social worker. The unit also has access to a dietitian and occupational therapist. Great emphasis is placed on preventive work, where the lifestyle clinic is the hub, and we also offer assistance with risk use and addiction issues. Long-term planning for continuity and stability for patients is essential, where our rehabilitation coordinator plays a vital role in helping patients move towards meaningful lives.

Qualifications

We are looking for a licensed nurse, preferably with further education as a district nurse, or a trained nurse with extensive experience as a nurse. Previous experience working in primary care or medical diabetes care is an advantage for this position. We can offer development opportunities in diabetes care if there is interest. We prefer that you have a broad base in your professional experience within health care that has developed you into an independent and professionally secure person while retaining the humility to continue learning. Acting professionally, being communicative, and having a good attitude are essential for us. You want to work for the best interests of the patients through treatment, counseling, and health-promoting work.

Preferably, you have experience with the Cosmic journal system that we use in our daily work. You also master the Swedish language in both speech and writing.

Employment Conditions

The position is a permanent full-time employment with a start date according to agreement.
