About the job

Continuum Care and Nursing is a private care provider in home care and home healthcare in Västerås.

Working in home healthcare means helping and supporting people with health and healthcare services in their ordinary homes.

In Home Healthcare, you work both independently and in teams. The tasks include team collaboration with other nurses, occupational therapist, physiotherapist, and home care. As a nurse in home healthcare, you work preventively to promote the patient's health and well-being and are responsible for medical assessments, documentation, and registration in various quality systems.

Working with us involves both direct and telephone contact with patients, relatives, and colleagues in home care, as well as documentation in our patient journal system. You will have close cooperation with home care, most of your working hours will be based at the same office as the home care group.

Some of your tasks include sampling, injections, dressings, assessments of the patient's condition. You will also take care of patients with end-of-life care decisions. You plan and follow up on nursing interventions. Documentation is done in Cosmic, planning is done in Epsilon.

In your daily work, you supervise and delegate as a caregiver. As a nurse, you have a great responsibility to contribute to constantly developing and improving the operation.

You will use Continuum's own cars during work and operate from our premises in Västerås.

We are looking for you who

We are looking for you who are a licensed nurse. A driver's license and driving experience are required for the position.

As a person, you are empathetic and have an easy time meeting people who, for various reasons, need our services. It is also important that you are service-oriented and have a strong desire to contribute to the best possible care for our patients. You need to be comfortable working independently.

It is advantageous if you have previous experience in similar work and have further education as a district nurse or a specialist education in elderly care (geriatrics). We prefer that you have documentation experience in Cosmic and that you are familiar with Pascal and Senior Alert.
