Hospitals in the West is an administration within the Region of Västra Götaland and consists of four hospitals – Alingsås Hospital, Angered Primary Care Hospital, Högsbo Primary Care Hospital, and Kungälv Hospital. Approximately 3,000 employees work with us every day to provide good and safe specialist healthcare of the highest quality.
Description
The Care Coordination team consists of 6 people who work with internal and external parties regarding the IT service SAMSA, the admission and discharge process, and care coordination.
Organizationally, the team belongs to the Rehabilitation Unit, along with occupational therapy, physiotherapy, speech therapy, counselors, dietitians, the stroke home rehabilitation team, the mobile palliative team, and the pain unit. The Rehabilitation Unit is part of the business area Geriatrics, Neurology, and Rehabilitation within Hospitals in the West.
Job Responsibilities
We are looking for you who want to work in our wonderful care coordination team and welcome you to a learning and developing position!
The team works towards the somatic care wards, the emergency department, and the ICU, working in various ways to enable a safe and secure admission and discharge process. As an employee in care coordination, you will be a knowledge resource for the admission and discharge process towards care wards, work extensively with and in the IT service SAMSA, and collaborate with contacts in regional and municipal primary care as well as social services.
The team often works out on care wards in contact primarily with healthcare staff but also with patients and other internal units at the hospital. The team also works with management on directed assignments such as investigations and compilation of statistics related to the admission and discharge process, and has a high level of participation in groups and issues concerning care coordination.
We work and communicate extensively via the IT service SAMSA, email, care meetings, and phone. The team has many practical tasks such as booking planning meetings, coordinating hospital-wide care meeting equipment, training healthcare staff in the IT service SAMSA, and performing other tasks related to the admission and discharge process. Some tasks are recurring and predictable, while others may be unforeseen, on short notice, and change continuously based on the hospital's and operations' current needs.
We work with continuous development and improvement of the operations, where all our employees have an important role.
You will be employed in your core profession.
Qualifications
Requirements:
- Adequate education for the assignment (core profession within healthcare or equivalent).
- At least a couple of years of professional experience from healthcare, municipal operations, or equivalent (e.g., social services).
- Experience handling the admission and discharge process and knowledge of guidelines and routines within the Region of Västra Götaland.
- Good knowledge and understanding of HSL (Healthcare Act) and SoL (Social Services Act) and their significance in care and support work.
- Knowledge of the IT service SAMSA and at least one year of experience working in SAMSA.
- Documented experience of collaboration in complex care processes.
- Ability to analyze and solve problems in complex care processes.
- Good communication skills in speech and writing, as well as a professional approach.
- Attention to detail and ability to work in a patient-safe manner.

About the Position
Daytime Monday - Friday.
Permanent, start as soon as possible, according to agreement.
