We are looking for a social worker (counselor) on a project-based contract for the South Karlstad Primary Care Centre.
Your workplace
South Karlstad Primary Care Centre, together with the Medical Clinic at CSK and Karlstad Municipality, has received project funding to develop psychosocial support for frail elderly patients with palliative care needs over the course of one year.
The support should be offered as soon as a life-threatening condition is identified and should also be available in the home environment. Interventions can be directed directly to the patient or to people in the patient's closest network. The project aims to investigate how psychosocial support can follow the elderly palliative patient across our organizational boundaries. The goal is also to highlight how we can best organize ourselves after the project period ends.
The project manager is located at the Elderly Care Team Karlstad, and we operate from the Resource Centre located next to the City Garden in Karlstad. We are open to combining this role with another permanent position, but we require you to work at least 50 percent on the project. The practical setup will be agreed upon with the project manager and other team members.
Your responsibilities
Here, you are expected to be actively involved and, together with employees from Karlstad Municipality, explore, test, and shape working methods that benefit the target group. There will be a steering group and a project manager who will provide backing and continuously monitor the work. Additional support, such as training or supervision, will be offered based on need.
The project involves optimizing existing resources by "picking up" on each other's efforts based on what the patient wants and needs, rather than necessarily based on what fits within care levels or organizational boundaries. Collaboration is crucial for early patient identification and creating a cohesive link based on the individual's needs and wishes. Your responsibilities include:
- Ensuring clear and accessible contact routes for patients, relatives, and employees.
- Participating in planning and collaboration around the patient where psychosocial support is needed.
- Offering advisory and supportive conversations, for example regarding economy and legal matters.
- Listening, and if necessary, representing and guiding the patient and relatives further.
- Collaborating with and supporting existing teams and operations that already meet the target group.
- Identifying and offering digital support and tools that work both at home and during transitions in care.
Your skills and competencies
We are looking for you who have a degree in Social Work (Social Worker/Socionom) or another education that the employer deems equivalent. Experience with difficult conversations and/or working in home environments is considered meritorious.
As a person, you are secure, professional, and have a problem-solving approach. You enjoy collaborating with others, are flexible, take initiative, and often come up with new ideas in work-related matters.
Since travel is involved in the position, you must have a valid Category B driver's license.
