Do you want to contribute to creating safety, quality, and continuity for our users in Haninge Mitt? We are now looking for an engaged coordinator for our home care team, based at our centrally located office in Haninge, a workplace with good transport links and a pleasant working environment.
Your role
As a coordinator within home care, you play a central role in ensuring that our services are planned, staffed, and followed up based on benefit decisions and implementation plans. You plan and lead the daily work, support the nursing staff, and collaborate closely with the operations manager to create safe and high-quality care. The role involves daily contact with clients, relatives, and external actors, as well as responsibility for documentation, onboarding of new staff, and administrative tasks. This role is suitable for you if you enjoy combining planning, leadership, and operational work.
Areas of responsibility
- Plan and staff home care services with a focus on quality and continuity
- Allocate clients, contact persons, and tasks
- Support and follow up on documentation in the Lifecare system
- Lead daily planned work and chair reporting meetings
- Support nursing staff with home care interventions when needed
- Onboard new staff and collaborate with other coordinators
- Handle orders, invoices, and reporting to the operations manager
- Ensure routines for key cabinets, phones, and transport
- Be available for clients, relatives, and employees
- Report deviations and transition time to the fee handler
Qualifications
Requirements:
To succeed in this role, we see that you have a relevant healthcare education, preferably as a nursing assistant, and several years of experience working in home care. You are confident in both nursing care and home care working methods. You have good digital competence with experience in documenting in operational systems. Since the role involves both collaboration and leadership, it is required that you communicate clearly and have the ability to organize and structure work efficiently.
Merits:
It is an advantage if you have previous experience from a coordinating role or have worked in a function with elements of team leadership. Experience in leading, distributing tasks, and supporting colleagues in daily work is seen as a significant plus and gives you good prerequisites for quickly getting into the role.

You have a professional attitude and work with economic awareness, while being engaged and flexible in everyday life. You contribute to a good work environment through your calmness, your ability to collaborate, and your focus on quality in every part of the work.
