Here, approximately 45 employees work in cross-professional general medical teams to provide our approximately 14,000 registered patients with good, close, and accessible care. Important actors for the patient interact through teamwork, where continuity is our cornerstone. The atmosphere at the health center is described as structured, welcoming, learning-oriented, and pleasant by both colleagues and students.

The patient-focused work is based on medical assessments and prioritizations, where we work knowledge-based with evidence-based treatments. We actively work to develop our treatment efforts for increased psychosocial health, good living habits, and to prevent long-term sick leave as well as promote a quick return to work.

Come and try out primary care; we welcome you even without specific primary care experience for a temporary position with us. We offer a temporary position at 75% for six months.

Your responsibilities

With us, you work both independently and in teams around the patient. The work is varied, broad, and educational, where we strive to use your and our shared knowledge, experiences, and areas of interest based on the patients' needs.

You work within a broad generalist area and use your knowledge to meet and care for patients of all ages, with great variation in care needs, disease patterns, and functional capacity. You have close collaboration with your nursing colleagues and other professions at the clinic, where you handle both scheduled and urgent visits, coordination cases, and can adjust when reprioritization is needed. Within the group, there are specialist competencies in living habits, diabetes, heart failure, asthma/COPD, osteoporosis, and incontinence, where we strive to use competencies as correctly as possible in the patient-focused work.

Your responsibilities consist largely of telephone advice and clinic work. You make assessments, triage, and perform both investigative, treating, nursing, and medical tasks. You are responsible for assessing care needs, triaging, and giving advice via telephone through TeleQ as well as handling digital care cases via the 1177 e-service.

The clinic work includes tasks such as vaccinations, medication administration, examinations, and wound dressings. You coordinate, plan, and perform nursing and medical measures in accordance with the patient's care plan. In some cases, you act as a permanent care contact for patients who need coordination, where you form an important part of the team around the patient based on individual needs and care plans.

Who are you?

You are a district nurse or a licensed nurse. We would like you to have experience with telephone advice and coordination of patients' care needs.

You want, together with us, to work for good care of high quality, and you have an easy time creating good meetings with both patients and coworkers.

We place great emphasis on your personal qualities and expect you to take responsibility and contribute with engagement and joy in work. As a person, you are engaged, solution-focused, and responsible.

You communicate clearly both orally and in writing and have good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference for Languages (CEFR).

A background check through extracts from the criminal record register is conducted before a decision on employment is made.
