Here, approximately 45 employees work in interdisciplinary general medical teams to provide our approximately 14,000 registered patients with good, close, and accessible care. Key actors important to the patient interact through teamwork, where continuity is our cornerstone.
The health center is characterized by dedicated and solution-oriented employees who, with professionalism and knowledge, take responsibility for both the development of the business and daily work with the patient in focus. We drive improvement initiatives initiated by employees to create more efficient care processes, better continuity, more person-centered and easily accessible care, along with a sustainable work environment. The patient-focused work is based on medical assessments and prioritizations, where we work with evidence-based treatments. We actively work to develop our treatment interventions for increased psychosocial health, good living habits, and to prevent long-term sick leave, as well as to promote a quick return to work.
Your responsibilities
Here, you work both independently and in teams around the patient. The work is varied, broad, and educational, where we take advantage of your knowledge, experience, and areas of interest based on the patients' needs.
You work within a broad generalist area and use your knowledge to meet and care for patients of all ages, with great variation in care needs, disease patterns, and functional capacity. You have close cooperation with your nursing colleagues and other professions at the clinic. You will handle both planned and urgent visits, coordination, and should be able to adapt when reprioritization is needed. Within the group, there are specialist competencies in lifestyle, diabetes, heart failure, asthma/COPD, osteoporosis, and incontinence, where we strive to use competencies as correctly as possible in the patient-focused work.
Your responsibilities consist largely of telephone advice and clinic work. You make assessments, triage, and perform both investigative, treating, nursing, and medical tasks. You are responsible for assessing care needs, triaging, and providing advice via telephone through TeleQ, as well as handling digital care cases via the 1177 e-service.
Clinic work includes tasks such as vaccinations, medication administration, examinations, and wound dressings. You coordinate, plan, and perform nursing and medical measures in accordance with the patient's care plan. In some cases, you act as a permanent care contact for patients in need of coordination, where you form an important part of the team around the patient based on individual needs and care plans.
Working hours are scheduled during daytime on weekdays, excluding holidays.

You communicate clearly both verbally and in writing and have good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference for Languages (CEFR).
A background check through an extract from the criminal record register will be conducted before a decision on employment is made.
