Job Responsibilities

Are you a confident and dedicated nurse who wants to work with a holistic perspective and high quality in care? We are now looking for a Care Manager Nurse who can contribute to patient safety and high quality of care for patients in ordinary housing within home healthcare.

Within Landskrona Omsorg AB, there is an HSL group with extended delegations handled by the home healthcare nurses. We are co-located at Kungsgatan 15 together with several professions, which strengthens collaboration and quality for patients in home healthcare, LSS (Support and Service for Persons with Functional Impairments), and care and nursing homes.

We expect everyone in our team to ensure that work is characterized by an ethical approach and carried out in accordance with current legislation and guidelines. The assignment includes traditional tasks for a Care Manager Nurse within municipal primary care, including but not limited to:

• Assess, plan, implement, and follow up on care interventions for patients with complex needs within municipal primary care.
• Collaborate with internal teams and external actors (inpatient care, primary care, ambulance) to ensure a cohesive care process.
• Provide guidance, support, and enthusiasm to care staff, relatives, and other caregivers to promote quality and security in care.
• Ensure correct documentation and reporting according to laws, guidelines, and patient safety requirements.
• Coordinate with medical technology and patients with complex care needs.

Qualifications and Merits

We are looking for you who are confident in your professional role and have a professional and respectful approach to treatment. You work in a structured manner, take responsibility for your work, and have good collaboration skills with an ease for creating and maintaining good relationships. As a person, you are solution-oriented and have a genuine commitment to quality and patient safety. We also see that you work methodically and with a scientific approach.

Requirements:

• Licensed nurse.
• Fluent Swedish in speech and writing.
• Category B driver's license for manual transmission (required).

Preferred Qualifications:

• Education and/or experience in nursing work and medical technology.
• Trained nurse (district nurse, specialist nurse with a focus on elderly care, or general nurse).
• Experience with documentation and work in digital medical record systems.
• Experience working in municipal care, palliative care, inpatient care, and/or primary care.
• Experience collaborating with other healthcare providers within municipal and regional care.
• Experience in treating different cultures and the cultural differences this entails.
