LSS Case Manager – A role where you truly make a difference

About the role

You will become part of an engaged team consisting of three colleagues and a senior social psychiatry case manager. You are responsible for your own cases and work independently, with close support from colleagues. You support each other in various cases and meet regularly for supervision, experience sharing, and development of working methods. The unit also includes assistance case managers within SoL (Social Services Act), an SVU team, and a health promotion team, creating good opportunities for collaboration. In this role, you meet people in different life situations. You receive applications, investigate needs, and make decisions regarding interventions according to LSS (Act concerning Support and Service for Persons with Certain Functional Impairments). You collaborate both internally and externally, for example with service providers and healthcare actors.

Who we are looking for

You take responsibility in your role while contributing to the whole. You work in a structured and independent manner but value collaboration and knowledge sharing.

You meet people with respect, clarity, and sensitivity. You can make decisions even in complex situations and are confident in both conversations and documentation.

You have:

• A degree in Social Work
• At least 3 years of experience in authority work, preferably within LSS
• The ability to express yourself clearly and correctly in speech and writing
• A valid driver's license (Category B)

Experience with LSS investigations and work within LSS implementation, as well as training in alternative communication, motivational interviewing, or plain language, is meritorious.
