Case Manager for Elderly Care Procurement Unit, Vällingby
Stockholm – our capital city of millions of dreams, expectations, and ambitions. We work for everyone living here today and in the future. We are now looking for you who want to think big, new, and different with us – for the people of Stockholm.
Welcome to us
The Elderly Care Procurement Unit in Vällingby is part of the administration's department for elderly, disability, and social psychiatry. We work closely with municipal providers within home care, care and nursing homes, as well as LSS (Act for Support and Service for Persons with Certain Functional Impairments) operations. The organization is new and consists of two units working with the same approach and a shared responsibility for the entire department's mission. Collaboration and support between units are a natural part of the work. Our workplace is characterized by participation, development, and close collaboration where we help each other create a legally secure and high-quality operation.
Today, 27 case managers work with us, investigating needs, making decisions, and following up on measures according to the Social Services Act for people over 65. The procurement units are led by two unit managers, and within the units, there are also team leaders, case managers with special assignments as mentors/method developers, business controllers, transport service case managers, and administrative staff.
We offer you
As an employee at one of our elderly care procurement units, you are offered a workplace that values professionalism and legal security highly. You have the opportunity for flexible hours and wellness benefits. It is easy to find us as we are located in the district's administration building right near the metro station at Hässelby Torg. Our premises were newly renovated last summer, and we share offices, which creates extra closeness for support and guidance from colleagues.
Together, we create a joyful work environment while supporting each other and sharing our knowledge and experiences.
A greeting from your future manager and team leader
I am Linda, your future direct manager, and together with team leader Suzanna, we look forward to welcoming a new colleague to the team. For us, collaboration, engagement, and an open climate are important parts of the daily work. We believe in present leadership where you receive support, opportunities for development, and good conditions to thrive in your role.
Your role
In your role as a case manager, you are responsible for receiving applications according to the Social Services Act, investigating needs, making decisions, and following up on approved measures. You conduct home visits, office visits, and visits within inpatient care. You collaborate both internally within the administration and with external actors such as healthcare. Your work as a case manager is significant and multifaceted as you, in dialogue with the elderly person, work to find a solution for the unique individual's specific needs.
Your qualifications and experience
We are looking for you who have:
- a university degree in social work, or alternatively, a corresponding higher education that the employer finds relevant
- knowledge of current legislation within elderly care
- very good knowledge of Swedish, both spoken and written, is a requirement, as a large part of the tasks involve legally secure processing and documentation.
It is meritorious if you have:
- previous experience from the elderly care sector, for example from field studies, case management, as well as competence in areas such as mental health issues or substance abuse
- experience from Stockholm City's documentation systems Paraplyet and LifeCare.
Personal competencies
We are looking for you who are independent and confident in driving your work forward with responsibility and engagement. You have good communication skills and can build trusting relationships with users, relatives, and colleagues. As a person, you are proactive and see what needs to be done, while working in a structured manner and having the ability to plan, prioritize, and document your work carefully.
Other information
Before employment can be offered, you must present an extract from the police criminal record. The extract is ordered by the applicant via the police's e-service "work in the home with elderly and persons with disabilities". The extract must not be older than 6 months from the date of issue.
Interviews may be conducted continuously during the recruitment process.
This recruitment process includes a small work test as part of the selection. If you proceed in the process, you will receive more information and the opportunity to complete the work test.
Stockholm City works with competency-based recruitment aimed at considering each person's competence and thereby combating discrimination.
